64层螺旋CT冠状动脉成像诊断冠心病的敏感度、特异度及阴性预测值 被引量:1

Evaluation of the Sensitivity,Specificity and Negative Predictive Value of 64-slice Spiral CT Coronary Angiography in the Diagnosis of Coronary Heart Disease

摘要 目的分析64层螺旋CT冠状动脉成像(64-slice Spiral CT Coronary Angiography)诊断冠心病敏感度、特异度以及阴性预测值的价值研究。方法回顾我院于2018年1月至2020年1月间接收的疑似冠心病患者共86例作为研究对象,依据本次检查方式分成参照组、试验组,评价两组检查的准确性与CT冠状动脉成像对各分支血管异常的诊断效能。结果CT冠状动脉成像对血管分支诊断效能不同,敏感度、特异度、阳性预测值及阴性预测值最高的是左主干狭窄,最低的是左回旋支,差距明显(χ^(2)=14.058,14.363,16.325,15.021,P<0.05);试验组敏感度为94.76%、特异度为86.21%、阳性预测值为93.10%、阴性预测值为89.26%,参照组敏感度为76.19%、特异度为70.45%、阳性预测值为71.11%、阴性预测值为75.61%,由此可知,试验组敏感度明显较参照组更高,P<0.05;两种方法特异度、阴性预测值比较无明显变化,P>0.05。结论针对冠心病诊断选择64层螺旋CT冠状动脉成像诊断的精准性更高,可对临床诊断、治疗提供有利依据。 Objective To analyze the sensitivity,specificity and negative predictive value of 64-slice spiral CT coronary aniography in the diagnosis of coronary heart disease.Methods 86 patients with suspected coronary heart disease received in our hospital from January 2018 to January 2020 were reviewed and assigned to reference group and test group according to this examination method.The accuracy of the two groups and the diagnostic efficacy of CT coronary aniography for abnormal blood vessels in each branch were evaluated and analyzed and summarized.Results CT coronary imaging has different diagnostic performances for vascular branches.The sensitivity,specificity,positive predictive value and negative predictive value are the highest in left main stenosis,and the lowest in left circumflex artery,and the difference was significant(χ^(2)=14.058,14.363,16.325,15.021,P<0.05).The sensitivity of the test group was 94.76%,the specificity was 86.21%,the positive predictive value was 93.10%,and the negative predictive value was 89.26%.The sensitivity,positive predictive value,specificity,and negative predictive value of the reference group were 76.19%,71.11%,70.45%,and 75.61%.It can be seen that the sensitivity of the experimental group was significantly higher than that of the reference group,P<0.05;the specificity,negative predictive value of the two methods had no significant change,P>0.05.Conclusion The choice of 64-slice spiral CT coronary aniography in the diagnosis of coronary heart disease is more accurate,which can provide a favorable basis for clinical treatment.
