电针压痛点结合触激脊神经根治疗腰椎间盘突出症的临床观察

Observation on the Clinical Effect of Electroacupuncture Stimulation Tender Point with Stimulate Spinal Nerve Roots in Lumbar Disc Herniation

目的探讨电针压痛点结合触激神经根在治疗腰椎间盘突出症的临床效果。方法选取2020年1月至2021年12月在厦门市第三医院康复医学科治疗的120例腰椎间盘突出伴有坐骨神经痛患者,随机分为3组:电针压痛点组、触激脊神经根组、电针压痛点结合触激脊神经根组3组。电针压痛点予以0.32 mm×75 mm毫针针刺病变椎体区间压痛点,接G6805-1B型电针仪刺激,每次25 min,隔日1次,5次为1个疗程,连续2个疗程;触激脊神经根组予以0.32 mm×75 mm毫针触激病变腰椎脊神经根,稍提针尖,予以电针仪刺激,接G6805-1B型电针仪刺激,每次25 min,隔日1次,5次为1个疗程,连续2个疗程;电针压痛点结合触激脊神经根组予以0.32 mm×75 mm毫针触激病变腰椎脊神经根,稍提针尖,同时针刺病变椎体区间压痛点,接G6805-1B型电针仪刺激,每次25 min,隔日1次,5次为1个疗程,连续2个疗程。采用日本骨科协会JOA评分评估腰椎功能的改善程度,采用VAS线段评分法评价患者疼痛程度,通过直腿抬高试验反映坐骨神经卡压程度。结果治疗后3组在日本骨科协会JOA评分、VAS评分、直腿抬高试验、临床疗效上进行两两比较,结果显示电针压痛点组、触激神经根组比较,差异无统计学差异(P>0.05),而电针压痛点结合触激神经根组分别优于电针压痛点组、触激神经根组,差异有统计学意义(P<0.05)。结论电针压痛点结合触激神经根在缓解腰椎间盘突出伴有坐骨神经痛患者上能提高临床疗效,明显优于单一治疗。

Objective To observe the clinical efficacy of Electroacupuncture stimulation tender point with Stimulate spinal nerve roots of lumbar disc herniation.Methods 120 patients with lumbar disc herniation treated in the department of rehabilitation medicine of Xiamen Third Hospital from January 2020 to December 2021 were randomly divided into three groups:stimulation tender point group,stimulate spinal nerve roots group,stimulation tender point combined stimulate spinal nerve roots group.The stimulation tender point group pierce the specification of 0.32 mm×75 mm acupuncture needles tender point of diseased lumbar spine,stimulation with the electroacupuncture of model G6805-1B,25 minutes each time,once every other day,five times as one course of treatment,tuo consecutive courses.stimulate spinal nerve roots group pierce the specification of 0.32 mm×75 mm acupuncture needles Stimulates the spinal nerve roots of the diseased vertebra,lift the needle a little bit,stimulation with the electroacupuncture of model G6805-1B,25 minutes each time,once every other day,five times as one course of treatment,tuo consecutive courses.Stimulation tender point combined Stimulate spinal nerve roots group,pierce the Specification of 0.32 mm×75 mm acupuncture needles stimulates the spinal nerve roots of the diseased vertebra,lift the needle a little bit,pierce acupuncture needles tender point of diseased lumbar spine at the same time.stimulation with the electroacupuncture of model G6805-1B,25 minutes each time,once every other day,five times as one course of treatment,tuo consecutive courses.Observe use Japanese Orthopaedic Association JOA Assess the degree of improvement in lumbar spine function,use visualized analoglogue score VAS Assess the patient's pain level,use straight leg raise test reflects the degree of sciatic nerve entrapment.Results After treatment,Pairwise comparison of the 3 groups in comparison Japanese Orthopaedic Association(JOA),visualized analoglogue score(VAS),Assess the patient's pain level.The results showed that there was no statistically difference between the stimulation tender point group and Stimulate spinal nerve roots group(P>0.05),the efficacy of stimulation tender point combined stimulate spinal nerve roots group was better than that of the stimulation tender point group and stimulate spinal nerve roots group,differences was statistically significant(P<0.05).Conclusion Stimulation tender point combined stimulate spinal nerve roots can improve clinical efficacy for The treatment of lumbar disc herniation,significantly better than monotherapy.
