铸瓷高嵌体修复后牙大面积牙体缺损的远期效果及对咀嚼功能、牙龈状况的影响

Long-term effect of cast porcelain onlays in repairing of large defect of posterior teeth and its effect on chewing function and gingival condition

目的:探讨铸瓷高嵌体修复后牙大面积牙体缺损的远期效果及对咀嚼功能、牙龈状况的影响。方法:纳入2018年7月~2020年1月我院口腔科收治的120例后牙大面积牙体缺损患者,随机数字表法分为铸瓷高嵌体组与树脂填充组,每组60例。修复后随访24个月,比较2组修复成功率、临床效果、咀嚼效率、牙龈状况指标(菌斑指数、牙龈指数)及不良事件。结果:修复后24个月,铸瓷高嵌体组修复成功率、邻接关系、边缘密合、修复体完整性、牙龈健康均明显优于树脂填充组(P<0.05)。铸瓷高嵌体组咀嚼效率优于树脂填充组(P<0.05),菌斑指数及牙龈指数明显低于树脂填充组(P<0.05)。铸瓷高嵌体组不良事件发生率明显低于树脂填充组(P<0.05)。结论:铸瓷高嵌体后牙大面积牙体缺损的远期效果显著,能有效改善患者咀嚼功能及牙龈状况。

Objective:To discuss the long-term effect of cast porcelain onlays in repairing of large defect of posterior teeth and its effect on chewing function and gingival condition.Methods:A total of 120 patients with large defect of posterior teeth who were enrolled in the department of Stomatology of our hospital from July 2018 to January 2020.According to random number table method, the patients were divided into cast porcelain onlays group and resin filling group, with 60 cases in each group.All the patients were followed up for 24 months after repair, and the success rates of repair, clinical effect, chewing efficiency, gingival status indicators(plaque index, gingival index)and adverse events were compared between two groups.Results:The success rate of repair, adjacent relationship, edge tightness, prosthesis integrity and gingival health in cast porcelain onlays group was significantly better than those in resin filling group(P<0.05)24-month after repair.The chewing efficiency in cast porcelain onlays group was significantly higher than those in resin filling group(P<0.05).The plaque index and gingival index in cast porcelain onlays group was significantly lower than those in resin filling group(P<0.05).The adverse events rates in cast porcelain onlays group was significantly lower than those in resin filling group(P<0.05).Conclusion:The long-term effect of cast porcelain onlays in repairing of large defect of posterior teeth was significant, and it could effectively improve the chewing function and gingival condition of patients.
