卡泊芬净治疗泌尿系真菌感染一例

Caspofungin in the treatment of urinary fungal infection:a case report

卡泊芬净等棘白菌素类药物在尿液中的浓度较低且缺乏临床数据,其治疗泌尿系真菌感染的效果还存在争议。但对于耐唑类真菌感染、肾功能不全的患者,棘白菌素类药物可能是一种治疗选择。本文报道了1例临床药师参与的尿路假丝酵母菌感染的治疗过程,临床药师结合患者泌尿系感染症状、查阅国内外相关文献和指南,建议临床医师给予卡泊芬净,同时实施药学监护,保障了患者的安全合理用药。

The efficacy of echinocandins such as Caspofungin in the treatment of urinary fungal infections has been controversial because this kind of anti-fungals are at low concentrations in urine and insufficient clinical data has provided to support this indication.However,echinocandins may also be a therapeutic option for patients with triazole-resistant fungal infections combined with renal insufficiency.This paper reported the pharmaceutical care provided by a clinical pharmacist in the treatment of urinary tract infection caused by Candida.Based on urinary tract infection symptoms of the case and review on relevant international and domestic literatures and guidelines,the clinical pharmacist suggested clinicians to administer Caspofengin,and implemented pharmaceutical care to ensure the safe and rational drug use of the patient.
