晚期胆囊癌肝转移转化治疗后R0切除一例

R0 resection after transformation therapy for advanced gallbladder liver metastasis:A case report

摘要 目的通过循证医学为一例肝转移的晚期胆囊癌(gallbladder cancer,GBC)患者定制治疗方法。方法对湖州市第一人民医院2021年8月收治的1例晚期GBC伴肝转移、肝门部及胰头后多发淋巴结转移患者评估病情,根据患者基本情况、基因检测结果,结合国内外医学文献资料、多学科MDT讨论,确定转化治疗方案,实施根治性手术切除。结果经3周期GEMOX方案(奥沙利铂100 mg/m^(2) d1+吉西他滨1000 mg/m^(2) d1+d8,21 d/周期)化疗联合免疫抑制剂治疗(特瑞普利单抗240 mg,21 d/周期)后,影像学复查达到部分缓解,存在R0切除可能。进行GBC根治性手术(胆囊切除+肝方叶切除+区域淋巴结清扫),达到R0切除。患者至今随访9个月未见复发。结论对于晚期GBC病例,在评估不可切除后,可尝试化疗联合免疫抑制剂治疗进行转化治疗,然后再实施根治性手术切除。
