持续负压封闭引流联合改良腓肠肌内侧头肌皮瓣治疗胫骨骨髓炎

Clinical analysis of VSD continuous negative pressure drainage combined with modified medial gastrocnemius head myocutaneous flap for treatment of tibial osteomyelitis

目的探讨持续负压封闭引流联合改良腓肠肌内侧头肌皮瓣治疗胫骨骨髓炎的方法及临床疗效。方法2015年1月-2021年1月,对16例胫骨骨髓炎患者采取持续负压封闭引流联合改良腓肠肌内侧头肌皮瓣转位覆盖修复的综合治疗,通过临床观察及跟踪随访,总结治疗效果。结果16例切口均愈合,院后随访2~18个月,平均9个月,骨髓炎全部治愈,局部皮肤无红肿、发热、软组织窦道等情况,未发现复发迹象。结论应用持续负压封闭引流联合改良腓肠肌内侧头肌皮瓣治疗胫骨骨髓炎,二者联合能够相互催化,充分放大各自优点,安全、可靠、效果显著,是一种有效的治疗方法,值得临床推广应用。

Objective To study the clinical efficacy of VSD continuous negative pressure drainage combined with modified medial gastrocnemius head myocutaneous flap for treatment of tibial osteomyelitis.Methods From January 2015 to January 2021,a total of 16 tibial osteomyelitis patients treated with the comprehensive treatment of VSD with combination of modified medial gastrocnemius head myocutaneous flapwas included in this study.The clnical efficacy of the treatment method was analyzed through clinical observation and follow-up.Results The incision of 16 patients healed as scheduled and were followed for 2 to 18 months and for 9 months on average.All osteomyelitis was cured,with no local skin swelling,fever,soft tissue sinus tract,and no signs of recurrence were found.Conclusion The application of VSD continuous negative pressure attraction drainage combined with modified medial gastrocnemius head myocutaneous flapis effective for tibial osteomyelitis.The combination can catalyze each other,can fully amplify their advantages,good safety and reliability,and effective effect,which is worthy of clinical promotion and application.
