摘要
目的:探讨腹腔镜术后患者中性粒细胞与淋巴细胞比值(NLR)对患者深静脉血栓形成(DVT)的预测作用。方法:选取在医院行腹腔镜手术的137例患者,根据患者术后发生深静脉血栓的情况将其分为血栓组(6例)和无血栓组(131例)。采用相关性分析明确NLR与血栓形成相关指标的关系。通过受试者工作特征(ROC)曲线,分析NLR对患者术后深静脉血栓形成的预测效能。采用单因素和多因素回归分析明确NLR对术后患者深静脉血栓形成的独立预测作用。结果:血栓组与无血栓组相比,深静脉血栓形成的患者其纤维蛋白原水平更高,差异有统计学意义(t=-0.873,P<0.05)。术后NLR水平与纤维蛋白原水平存在相关性(r=0.254,P<0.05)。术后NLR预测血栓组患者深静脉血栓形成的ROC曲线下面积(AUC)为0.753,灵敏度、特异度分别为83.3%、68.7%;界值为2.76。Logistic回归分析中,NLR的水平高低对DVT的发生有独立预测作用(OR=10.208,P<0.05)。结论:腹腔镜术后NLR水平对患者深静脉血栓的形成有预测作用。
Objective:To investigate the predictive effect of neutrophil to lymphocyte ratio(NLR)on patients deep venous thrombosis(DVT)after laparoscopic surgery.Methods:A total of 137 patients who underwent laparoscopic surgery in hospital were enrolled in this study.And they were divided into thrombus group(6 cases)and non-thrombus group(131 cases)according to whether occurred DVT after surgery.The correlation analysis was adopted to confirm the relationship between NLR and related indicators of thrombosis.The receiver operating characteristic curve(ROC)was used to analyze the predictive efficiency of NLR on postoperative DVT.Univariate and multivariate regression analysis were adopted to confirm that the independent predictive effect of NLR on DVT of patients.Results:The level of fibrinogen in patients with DVT of thrombus group was significantly higher than that of non-thrombus group(t=-0.873,P<0.05).There was a correlation between NLR level and fibrinogen level after surgery(r=0.254,P<0.05).The area under curve(AUC)of ROC curve,sensitivity and specificity of postoperative NLR were 0.753,83.3%and 68.7%in predicting DVT of patients in thrombus group,respectively,and the cutoff value of that was 2.76.In Logistic regression analysis,the NLR level had independent prediction effect on the occurrence of DVT(OR=10.208,P<0.05).Conclusion:NLR level after laparoscopic surgery can predict the DVT formation of patients.
