不同材料髓腔固位冠修复大面积缺损磨牙疗效比较

Comparison of the Effects of Endocrown of Different Materials in Repairing Large Area Defective Molars

目的:探讨铸瓷和聚合瓷两种不同材料髓腔固位冠修复大面积缺损磨牙的临床效果。方法:选取2017年2月-2018年2月笔者医院口腔科收治的104例大面积缺损磨牙患者(共132颗患牙)进行研究。根据随机数字表法将患者分为聚合瓷组和铸瓷组。聚合瓷组52例(67颗患牙),行聚合瓷髓腔固位冠修复;铸瓷组52例(65颗患牙),行铸瓷髓腔固位冠修复,治疗后随访2年。比较两组临床修复效果、边缘密合度情况、修复后不良反应及修复满意度。结果:聚合瓷组临床修复优良率高于对照组,1年后边缘密合度、2年后边缘密合度均优于铸瓷组,不良反应发生率低于铸瓷组,咀嚼功能、边缘密合度、美观性满意度评分均高于铸瓷组,差异均有统计学意义(P<0.05)。结论:大面积缺损磨牙均可采用铸瓷和聚合瓷髓腔固位冠修复,相对于铸瓷,聚合瓷髓腔固位冠修复临床效果更好,修复满意度更高。

Objective To compare the clinical effects of endocrown made of cast porcelain and polymer porcelain in repairing large area defective molars.Methods A total of 104 patients with large area defective molars (132 teeth) treated in the stomatology department of our hospital from February 2017 to February 2018 were studied.According to the random number table,the patients were randomly divided into polymeric porcelain group and cast porcelain group.52 cases (67 teeth) in the polymeric porcelain group were restored with polymeric porcelain crown of the endocrown type,and 52 cases (65 teeth) in the cast porcelain group were repaired with cast porcelain.The patients were followed up for 2 years.The clinical repair effect,marginal fitness,adverse reactions after repair and repair satisfaction were compared between the two groups.Results The excellent and good rate of clinical restoration in the polymeric porcelain group was higher than that in the control group,and the marginal fitness 1 year and 2 years later in the polymeric porcelain group were better than those in the cast porcelain group,and the incidence of adverse reactions in the polymeric porcelain group was lower than that in the cast porcelain group.The scores of masticatory function,marginal fitness and aesthetic satisfaction in the polymeric porcelain group were significantly higher than those in the cast porcelain group (P<0.05).Conclusion The large area defective molars can be repaired with cast porcelain and polymeric porcelain crown.Compared with cast porcelain,polymeric porcelain crown has better clinical effect and higher satisfaction.
