布地奈德联合鼻内镜手术治疗慢性鼻-鼻窦炎伴变应性鼻炎患者的效果

Effects of Budesonide combined with Nasal endoscopic surgery in patients with chronic rhinosinusitis and allergic rhinitis

目的:观察布地奈德联合鼻内镜手术治疗慢性鼻-鼻窦炎(CRS)伴变应性鼻炎(AR)患者的效果。方法:回顾性分析2020年1月至2021年1月该院收治的64例CRS伴AR患者的临床资料,按照治疗方法不同分为对照组和观察组各32例。对照组采用鼻内镜手术治疗,观察组采用布地奈德联合鼻内镜手术治疗,比较两组临床疗效、治疗前后鼻窦炎症状[鼻腔鼻窦结局测试(SNOT-20)]评分、鼻内镜Lund-Kennedy评分、炎性因子[白细胞介素(IL)-5、IL-32、肿瘤坏死因子-α(TNF-α)]水平及并发症发生率。结果:观察组治疗总有效率为93.75%,高于对照组的75.00%,差异有统计学意义(P<0.05);治疗后,观察组SNOT-20评分和鼻内镜Lund-Kennedy评分均低于对照组,差异有统计学意义(P<0.05);观察组血清IL-5、IL-32、TNF-α水平均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为6.24%,低于对照组的31.25%,差异有统计学意义(P<0.05)。结论:布地奈德联合鼻内镜手术治疗CRS伴AR患者,可提高临床疗效,缓解鼻窦炎症状,降低炎性因子水平和并发症发生率,其效果优于单纯鼻内镜手术治疗。

Objective:To observe effects of Budesonide combined with Nasal endoscopic surgery in patients with chronic rhinosinusitis(CRS) and allergic rhinitis(AR).Methods:The clinical data of 64 CRS patients with AR admitted to this hospital from January 2020 to January 2021 were retrospectively analyzed,and were divided into control group and observation group according to different treatment methods,32 cases in each group.The control group received nasal endoscopic surgery,while the observation group received Budesonide combined with nasal endoscopic surgery.The clinical effects,the sinusitis symptom score[sinonasal outcome test(SNOT-20)]score,the endoscopic Lund-Kennedy score,the inflammatory factor levels[interleukin(IL)-5,IL-32,tumor necrosis factor-α(TNF-α)]and the incidence of complications were compared between the two groups.Results:The total effective rate of treatment in the observation group was 93.75%,which was higher than 75.00%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the SNOT-20 score and the endoscopic Lund-Kennedy score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of IL-5,IL-32 and TNF-αin the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 6.24%,which was lower than 31.25%in the control group,and the difference was statistically significant(P<0.05).Conclusions:Budesonide combined with nasal endoscopic surgery in the treatment of the patients with CRS and AR can improve the clinical effects,relieve the sinusitis symptoms,and reduce the levels of inflammatory factors and the incidence of complications.Moreover,it is superior to single nasal endoscopic treatment.
