不同服药时间对老年高血压患者血压的影响

摘要 目的:分析老年高血压患者在不同时间服用药物对血压和血压昼夜节律的影响。方法:选取2021年2月-2022年1月北京市顺义区医院综合病房收治的老年高血压患者148例作为研究对象,所有研究对象入院后均进行24 h血压动态监测,观察组74例患者夜间服用氨氯地平、奥美沙坦治疗,对照组74例患者晨起服用氨氯地平、奥美沙坦治疗,治疗2周后进行动态血压监测,比较治疗效果。结果:治疗后,观察组夜间平均舒张压、夜间平均收缩压、24 h平均收缩压低于对照组,差异有统计学意义(P<0.05);治疗后,两组杓型高血压患者均增多,观察组杓型高血压患者多于对照组,观察组非杓型高血压患者少于对照组,差异有统计学意义(P<0.05);观察组的血压变异性相关指标低于对照组,差异有统计学意义(P<0.05)。结论:在老年高血压患者的药物治疗中,通过夜间服用降压药物氨氯地平和奥美沙坦,可有效控制全天血压水平和纠正非杓型血压节律,且可以降低患者的血压变异性。 Objective:To analyze the effects of different medication time on blood pressure and blood pressure circadian rhythm in elderly hypertensive patients.Methods:A total of 148 elderly hypertensive patients admitted to the general ward of Beijing Shunyi District Hospital from February 2021 to January 2022 were selected as the research subjects.All subjects underwent 24-h ambulatory blood pressure monitoring after admission.74 patients in the observation group were treated with amlodipine and olmesartan at night,and 74 patients in the control group were treated with amlodipine and olmesartan in the morning.After 2 weeks of treatment,ambulatory blood pressure monitoring was performed to compare the therapeutic effect.Results:After treatment,the nighttime average diastolic blood pressure,nighttime average systolic blood pressure,and 24-hour average systolic blood pressure in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the number of dipper-type hypertension patients in the two groups increased.The number of dippertype hypertension patients in the observation group was more than that in the control group,and the non-dipper-type hypertension patients in the observation group were less than those in the control group,and the difference was statistically significant(P<0.05).The blood pressure variability-related indexes in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:In the drug treatment of elderly hypertensive patients,taking the antihypertensive drugs amlodipine and olmesartan at night can effectively control the blood pressure level throughout the day and correct the non-dipper blood pressure rhythm,and can reduce blood pressure variability in patients.
