健脾补肾化瘀法治疗早期先兆流产合并宫腔积液的临床观察

Clinical Observation of Invigorating the Spleen,Invigorating the Kidney and Removing Blood Stasis in the Treatment of Early Threatened Abortion Complicated with Uterine Effusion

目的:观察健脾补肾化瘀法治疗早期先兆流产合并宫腔积液的临床疗效。方法:选取2020年1月-2021年6月广州市荔湾区中医医院中医生殖科收治的早期先兆流产合并宫腔积液患者60例为观察对象,按随机数字法分为观察组与对照组,各30例。观察组给予健脾补肾化瘀中药汤剂治疗,对照组给予固肾安胎丸+黄体酮治疗,比较两组孕酮激素水平、β绒毛膜促性腺激素(β-HCG)、糖类抗原125(CA-125)血清指标、宫腔积液改善情况与治疗效果。结果:观察组孕酮激素及血β-HCG水平高于对照组,差异具有统计学意义(P<0.05);治疗后,观察组CA-125、宫腔积液平均径线水平低于对照组,差异具有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。结论:在早期先兆流产合并宫腔积液患者的治疗中,应用纯中药健脾补肾化瘀法能够有效改善患者的血β-HCG及孕酮激素水平,同时还可以降低患者CA-125的血清指标,缩小宫腔积液的面积,提高治疗总有效率。

Objective:To observe the clinical effect of invigorating the spleen,invigorating the kidney and removing blood stasis in the treatment of early threatened abortion complicated with intrauterine effusion.Methods:From January 2020 to June 2021,60 patients with early threatened abortion combined with intrauterine effusion who were admitted to the Department of Reproductive Medicine,Liwan District Hospital of Traditional Chinese Medicine,Guangzhou City,were selected as observation objects.They were divided into observation group and control group according to random method,30 cases in each group.The observation group was given Chinese medicine decoction for invigorating the spleen,invigorating the kidney and removing blood stasis,and the control group was given the treatment of Gushen Antai Pill+progesterone.The levels of progesterone hormone,β-chorionic gonadotropin(β-HCG),serum indexes of carbohydrate antigen 125(CA-125),the improvement of intrauterine effusion and the therapeutic effect were compared between the two groups.Results:The levels of progesterone hormone and bloodβ-HCG in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of CA-125 and the average diameter of uterine effusion in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:In the treatment of patients with early threatened abortion and intrauterine effusion,the application of pure traditional Chinese medicine invigorating the spleen,invigorating the kidney and removing blood stasis can effectively improve the bloodβ-HCG and progesterone hormone levels of the patients.At the same time,it can also reduce the serum index of CA-125 in patients,reduce the area of uterine effusion,and improve the total effective rate of treatment.
