超声诊断甲状腺占位性病变的价值分析

Analysis of the Value of Ultrasonography in the Diagnosis of Thyroid Space-occupying Lesions

目的:分析超声诊断甲状腺占位性病变的价值及实用性。方法:选取2018年9月-2020年9月佛山市南海区第七人民医院收治的疑似甲状腺占位性病变患者100例为观察对象,均给予超声诊断与手术病理诊断,将手术病理诊断作为诊断甲状腺占位性病变的“金标准”。统计超声诊断灵敏度、特异度、阴性预测值、阳性预测值以及准确率,对比超声检查与手术病理诊断甲状腺占位性病变的分型检出率。结果:100例患者手术病理诊断确诊98例,超声确诊96例;超声准确率为96.00%,灵敏度为96.94%,特异度为50.00%,阳性预测值为98.96%,阴性预测值为25.00%;超声诊断炎性结节、滤泡性腺瘤、增生结节、乳头状癌、甲状腺囊肿的检出率与手术病理诊断比较,差异无统计学意义(P>0.05)。结论:超声具有较高的甲状腺占位性病变诊断准确率、灵敏度、特异度,分型检出率高,实用性强。

Objective:To analyze the value and practicability of ultrasonography in the diagnosis of thyroid space-occupying lesions.Methods:From September 2018 to September 2020,100 patients with suspected thyroid space-occupying lesions admitted to the Seventh People's Hospital of Nanhai District,Foshan City were selected as the observation objects.Ultrasound diagnosis and surgical pathological diagnosis were given.Surgical and pathological diagnosis was regarded as the"gold standard"for the diagnosis of thyroid space-occupying lesions.The sensitivity,specificity,negative predictive value,positive predictive value and accuracy of ultrasound diagnosis were analyzed.To compare the classification detection rate of ultrasonography and surgical pathology in diagnosing thyroid space-occupying lesions.Results:Of the 100 patients,98 were diagnosed by surgical pathology,and 96 were diagnosed by ultrasound.The accuracy of ultrasound was 96.00%,the sensitivity was 96.94%,the specificity was 50.00%,the positive predictive value was 98.96%,and the negative predictive value was 25.00%.There was no significant difference in the detection rate of ultrasound diagnosis of inflammatory nodules,follicular adenomas,hyperplastic nodules,papillary carcinomas and thyroid cysts compared with surgical and pathological diagnosis(P>0.05).Conclusion:Ultrasound has high diagnostic accuracy,sensitivity and specificity for thyroid space-occupying lesions,high classification detection rate and strong practicability.
