L-JQC质控图法在血液库存预警管理中的应用

Application of Levey-Jennings quality control(L-JQC) statistic graph in blood inventory early warning management

目的 将L-JQC质控图法应用于血液库存管理,以期对可能发生的涨库和低库存进行预警,指导血站合理调控采供血计划,保持血液库存在合理区间内。方法 收集2017年1月—2022年4月每日血液出库量及库存量,建立数据库。依据当日前推1周(1W)、2周(2W)、3周(3W)的日均出库量,计算各年度的库存指数(D_(1W)、D_(2W)、D_(3W)),统计D_(1W)、D_(2W)、D_(3W)的(x±s)及变异系数(cv),选择cv最小的库存指数均值(D_(x))设置库存指数中线。应用L-JQC质控原理和质控规则,通过观察D值所在区间,判断每天的血液库存状态。结果 D_(1W)、D_(2W)、D_(3W)的(x±s)及对应的cv分别为[(10.77±2.05)、19.03%][(10.71±1.98)、18.49%][(10.70±2.13)、19.91%]。选择cv最小的D_(2W)均值作为库存指数中线。平均库存指数为10.71天,预警库存指数上限、下限分别为14.67天和6.75天,失控库存指数上限、下限分别为16.65天和4.77天。结论 基于L-JQC的血液库存预警方法实用性强,操作简便,能够帮助判别库存状态,对失控库存发出预警,提醒管理者查找分析原因,尽早干预,是血液库存预警的一种有效管理工具。

Objective To apply the Levey-Jennings quality control(L-JQC) chart method to blood inventory management,in order to give early warning of possible rising and low inventory,guide blood stations to reasonably regulate blood collection,and supply plans,and maintain blood inventory within a reasonable range.Methods By collecting the daily blood delivery volume and inventory from January 2017 to April 2022,a database was established.According to the daily average delivery volume of 1 week(1W),2 weeks(2W),and 3 weeks(3W) pushed before the current day,the inventory indexes(D_(1W),D_(2W),D_(3W)) of each year were calculated,and the(x ±s)s and cvs of D_(1W),D_(2W),and D_(3W) were counted,and the average value of the inventory index(D_(x)) with the smallest cv was selected to set the inventory index centerline.By applying the LJQC principle and quality control rules and observing the range of D value,the daily blood inventory status was judged.Results The( x±s)s and corresponding cvs of D_(1W),D_(2W) and D_(3W) were[(10.77 ± 2.05),19.03%],[(10.71 ± 1.98),18.49%]and[(10.70 ± 2.13),19.91%]respectively.By selecting the D_(2W) mean with the minimum cv as the centerline of the inventory index,the average inventory index was 10.71 days,the upper limit and lower limit of the early warning inventory indexes were 14.67 days and 6.75 days respectively,and the upper limit and lower limit of out-of-control inventory indexes were 16.65 days and 4.77 days respectively.ConclusionsThe blood inventory early warning method based on L-JQC is practical and easy to operate.It can help identify the inventory status,send an early warning to the out-of-control inventory,remind managers to find and analyze the reasons,and intervene as soon as possible.It is an effective management tool for blood inventory early warning.
