摘要
64岁男性,以皮疹、多处淋巴结肿大起病,尿液检查提示少量蛋白尿,无明显镜下血尿,血清肌酐升高,溶血性贫血,抗核抗体、抗双链DNA抗体阳性,补体下降,血清IgG4水平升高,肾活检病理提示IgG4相关间质性肾炎。结合临床及病理最终诊断伴狼疮样表现的IgG4相关疾病。
The patient was a male aged 64 year old with the following symptoms:rash and multiple enlarged lymph nodes.Laboratory examination results showed proteinuria,renal dysfunction,hemolytic anemia,antinuclear antibodies(ANAs)positivity,anti-dsDNA antibodies(AdsDNA)positivity,hypocomplementemia,and elevated serum IgG4 levels.Renal biopsy showed IgG4-positive-plasma cell infiltration in renal interstitium.Based on the clinical and pathological manifestations,the patient was final diagnosed as IgG4 related disease with SLE-like manifestations.
