二维超声与彩色多普勒超声对甲状腺癌的诊断价值及准确性分析 被引量:4

摘要 目的:探讨二维超声与彩色多普勒超声(彩超)应用于甲状腺癌诊断中的应用价值及准确性。方法:选取2019年1月—2022年1月莘县第三人民医院接收的疑似甲状腺癌患者86例作为此次分析对象(经病理检查已确诊为甲状腺癌80例),均进行二维超声与彩色多普勒超声检查,并以病理检查结果作为金标准,以此探究对比二维超声与彩色多普勒超声的临床诊断价值及准确性。结果:经二维超声诊断,86例患者中阳性72例,阴性14例;经彩色多普勒超声诊断,阳性患者79例,阴性患者7例。两种诊断方式具有相同的特异度;彩色多普勒超声诊断的准确度和敏感度均高于二维超声,差异有统计学意义(P<0.05)。二维超声诊断中血流丰富程度为Ⅰ级、Ⅱ级、Ⅲ级结果的患者分别为7例(9.72%)、29例(40.28%)、23例(31.94%),均少于彩色多普勒超声诊断的27例(34.18%)、43例(54.43%)、49例(62.03%),差异有统计学意义(P<0.05)。彩色多普勒超声诊断的周边无声晕、微钙化、低回声、颈部淋巴结检出率以及动脉收缩期峰值速度、阻力指数结果均高于二维超声诊断,差异有统计学意义(P<0.05)。结论:针对甲状腺癌的临床诊断,二维超声与彩色多普勒超声均具有一定的临床诊断价值,但相较于二维超声,彩色多普勒超声的诊断准确性相对较高,可为临床疾病的诊断提供重要的参考依据,值得推广运用。 Objective To investigate the application value and accuracy of two-dimensional ultrasonography and color Doppler ultrasonography(color Doppler ultrasonography)in the diagnosis of thyroid cancer.Methods Selected 86 patients with suspected thyroid cancer received by Shenxian Third People's Hospital from January 2019 to January 2022 were selected as the object of this analysis(80 cases of thyroid cancer were diagnosed by pathological examination),and all underwent two-dimensional Ultrasound and color Doppler ultrasonography.The pathological examination results were as the gold standard to explore and compare the clinical diagnostic value and accuracy of two-dimensional ultrasonography and color Doppler ultrasonography.Results According to two-dimensional ultrasound diagnosis,72 of 86 patients were positive and 14 were negative.By color Doppler ultrasonography,79 patients were positive and 7 were negative.The two diagnostic methods have the same specificity.The diagnostic accuracy and sensitivity of color Doppler ultrasound were higher than those of two-dimensional ultrasound,and the difference was statistically significant(P<0.05).The results of blood flow richness of gradeⅠ,ⅡandⅢin 2D ultrasound diagnosis were 7 cases(9.72%),29 cases(40.28%)and 23 cases(31.94%),respectively,which were lower than those of color Doppler ultrasonography 27 cases(34.18%),43 cases(54.43%)and 49 cases(62.03%),the difference was statistically significant(P<0.05).The results of peripheral silent halo,microcalcification,hypoecho,cervical lymph node detection rate,peak velocity of arterial systole and resistance index in color Doppler ultrasound diagnosis were higher than those in two-dimensional ultrasound diagnosis,and the difference was statistically significant(P<0.05).Conclusion For the clinical diagnosis of thyroid cancer,both two-dimensional ultrasonography and color Doppler ultrasonography have certain clinical diagnostic value.The diagnosis of disease provides an important reference and is worthy of popularization and application.
