基于磁共振DTI联合SWI在早产儿脑损伤诊断中的价值研究

摘要 目的:探讨磁共振弥散张量成像(diffusion tensor imaging,DTI)联合磁敏感加权成像(susceptibility weighted imaging,SWI)在早产儿脑损伤诊断中的价值。方法:选取2020年1月—2021年12月宁夏回族自治区人民医院新生儿科诊断为早产儿缺氧缺血性脑损伤(hypoxic-ischemic brain damage,HIBD)的43例患儿作为临床研究对象(设为研究组),并选取同期出生的43名正常的早产儿(设为对照组),两组均采用磁共振常规扫描序列及DTI扫描,研究组加扫SWI,应用DTI技术对临床HIBD患儿感兴趣区各向异性分数进行差异性分析,并对比SWI与常规磁共振序列对出血的检出率。结果:HIBD患儿与正常早产儿除豆状核外的各感兴趣区FA值比较,差异有统计学意义(P<0.05)。SWI较常规序列可更敏感地显示出血灶,差异具有统计学意义(P<0.05)。结论:DTI联合SWI技术可在早期对HIBD患儿进行诊断及评估,SWI较常规序列更容易发现微出血。 Objective To investigate the value of magnetic resonance diffusion tensor imaging(DTI)combined with susceptibility weighted imaging(SWI)in the diagnosis of brain injury in premature infants.Methods A total of 43 children diagnosed with hypoxicischemic brain damage(HIBD)in premature infants from January 2020 to December 2021 in the Department of Neonatology,People's Hospital of Ningxia Hui Autonomous Region were selected as clinical research subjects(set as study group),and selected 43 normal premature infants born in the same period(set as the control group),both groups were treated with conventional MRI scan sequence and DTI,and the study group was scanned with SWI,and DTI technology was used to identify and analyze the area of interest in clinical HIBD children anisotropy scores were analyzed for differences,and the detection rates of hemorrhages by SWI and conventional magnetic resonance sequences were compared.Results There was significant difference in FA values of all regions of interest between HIBD children and normal premature infants except lenticular nucleus(P<0.05).SWI can more sensitively display the bleeding focus than conventional sequence,and the difference is statistically significant(P<0.05).Conclusion DTI combined with SWI technology can diagnose and evaluate children with HIBD at an early stage,and SWI is easier to detect microbleeds than conventional sequences.
