摘要
目的 回顾分析某院血培养阳性分离马耳他布鲁菌病例1例,提高临床医生对以附睾炎为首发症状的布鲁菌病的科学认识。方法 将阳性血培养瓶转种血平板、麦康凯平板等培养基,培养过夜。挑取目标菌落进行传统细菌鉴定,使用基质辅助激光解析电离飞行时间质谱仪(matrix assisted laser desorption ionization time of flight mass spectrometer, MALDI-TOF MS)进行鉴定。结果 某院分离的细菌为革兰阴性短杆菌,氧化酶、触酶、脲酶试验阳性,MALDI-TOF MS鉴定为马耳他布鲁菌。结论 通过对本例病例进行回顾,加深了临床医生对以附睾炎为首发症状的布鲁菌病的认识,避免了临床误诊、漏诊。
Objective To improve the clinicians′ scientific understandings on brucellosis with epididymitis as the first symptom by retrospectively analyzing a case of Brucella melitensis isolated from positive blood culture.Methods The positive blood culture bottle was subcultivated to the blood agar plate and Mac-Conkey agar plate, and cultured overnight.The target bacterial colonies were selected for traditional identification, and matrix assisted laser desorption ionization time of flight mass spectrometer(MALDI-TOF MS) was used for identification.Results The bacteria were gram negative bacillus brevis and positive for oxidase test, urease test or catalase test, and Brucella melitensis was identified by MALDI-TOF MS.Conclusion The understanding of clinicians on epididymitis as the first symptom of brucellosis is intensified through retrospective analysis on the case, avoiding the misdiagnosis of clinicians.
