云南省艾滋病抗病毒治疗医护人员职业生命质量现状及影响因素分析

Quality of working life and influencing factors of medical staff providing ART in Yunnan Province

目的了解艾滋病ART医护人员职业生命质量的现状及影响因素,为改善其职业生命质量,维护职业健康提供依据。方法2022年1月,将自编一般信息调查表、职业生命质量量表发布于云南省艾滋病ART信息管理系统,采用整群抽样的方法,评估云南省艾滋病ART医护人员职业生命质量现状。结果共收到有效问卷442份,职业生命质量得分(109.06±18.09)分,其中,职业生命质量较差89人(20.14%),中等282人(63.80%),较好71人(16.06%)。医生(t=-2.082,P<0.05)、发生过重大负性事件(t=-4.375,P<0.001)、治疗人数低于500人(t=2.581,P<0.05)、职业倦怠程度高(t=-11.828,P<0.001)的ART医护人员职业生命质量较差,从事ART时间4~6年(t=-4.467,P<0.001)的医护人员职业生命质量较从事10年及以上者差。结论云南省ART医护人员职业生命质量总分及各维度得分均处于中等水平,应该重点关注医生群体,关心医护人员家庭情况,缓解职业倦怠状况,同时,还需从工作制度、配套医疗资源等方面着手,改善ART医护人员的职业生命质量。

Objective To understand the status and influencing factors of quality of working life(QWL)among medical staff who provided ART.Methods In January 2022,the general information questionnaire and Quality of Working Life scale(QWL)were spread in the Yunnan ART management system.The QWL of the medical staff providing ART in Yunnan was assessed by cluster sampling.Results A total of 442 valid questionnaires were received,and the overall score of QWL was(109.06±18.09),including 89 in the worse quality group(20.14%),282 in the medium quality group(63.80%)and 71 in the better quality group(16.06%).The medical staff who were doctors(t=-2.082,P<0.05),experiencing severe adverse events(t=-4.375,P<0.001),providing service for less than 500 HIV/AIDS patients(t=2.581,P<0.05)and with a high degree of job burnout(t=-11.828,P<0.001)had a worse QWL.The staff engaging in ART for 4-6years had worse QWL than whom working over ten years.Conclusions The total QWL and its responding dimension scores were at a medium level for ART medical staff.Therefore,we should care on the medical staff and their family situation to reduce their job burnout.At the same time,we also need to improve the QWL by establishing an appropriate work schedule and supporting medical resources.
