老年糖尿病患者低血糖恐惧感现状及其影响因素分析

Status and influencing factors of fear of hypoglycemia in elderly patients with type 2 diabetes

目的了解老年糖尿病患者的低血糖恐惧感现状,并分析其影响因素。方法采用便利抽样法,选取2021年5—10月入住河南省人民医院内分泌科的256例患者为研究对象,应用一般资料问卷、中文版低血糖恐惧感调查表、认知融合问卷及糖尿病患者自我管理问卷对患者进行调查。采用单因素分析、多重线性回归分析方法分析患者低血糖恐惧感的影响因素。结果老年糖尿病患者的低血糖恐惧感得分为(26.78±8.91)分,认知融合得分为(35.64±9.34)分,自我管理行为得分为(43.16±12.92)分;老年糖尿病患者的低血糖恐惧感与认知融合得分呈正相关(P<0.01),与自我管理行为得分呈负相关(P<0.01);最近1个月发生低血糖的次数、年龄、认知融合、自我管理行为是老年患者低血糖恐惧感的影响因素(P<0.01)。结论老年糖尿病患者低血糖恐惧感处于中等水平,患者最近1个月发生低血糖的次数、年龄、认知融合、自我管理行为可以影响老年患者的低血糖恐惧感水平,建议依据影响因素采取适当措施缓解老年糖尿病患者的低血糖恐惧感水平。

Objective To explore the occurrence and influencing factors of fear of hypoglycemia in elderly patients with type 2 diabetes.Methods A total of 256 patients who were admitted to Endocrinology Department in Henan Provincial People's Hospital from May to October 2021 were selected as the research objects.The general information questionnaire,the Chinese version of the Hypoglycemia Fear Questionnaire,the Cognitive Fusion Questionnaire and the Diabetes Self-management Questionnaire were used to investigate the patients.Univariate analysis and multiple linear regression analysis were used to analyze the influencing factors of patients'fear of hypoglycemia.Results The score of fear of hypoglycemia in elderly patients with type 2 diabetes was(26.78±8.91).The cognitive fusion score was(35.64±9.34).The score of self-management behavior was(43.16±12.92).Fear of hypoglycemia was positively correlated with cognitive integration(P<0.01),and negatively correlated with self-management behavior(P<0.01)in elderly patients with diabetes.The frequency of hypoglycemia in the last month,age,cognitive integration and self-management were the main influencing factors of hypoglycemia fear(P<0.01).Conclusions The fear of hypoglycemia in elderly patients with type 2 diabetes is at a moderate level.The frequency of hypoglycemia in the last month,age,cognitive fusion,self-management are the influencing factors of fear of hypoglycemia in elderly patients with type 2 diabetes,and targeted intervention should be taken to reduce fear of hypoglycemia.
