腹腔镜与开腹治疗阑尾炎合并糖尿病患者的疗效对比分析

Comparative Analysis of the Curative Effect of Laparoscopy and Open Surgery in the Treatment of Appendicitis Complicated with Diabetes Mellitus

目的 分析对比腹腔镜与开腹治疗阑尾炎合并糖尿病患者的临床疗效。方法 选取急性阑尾炎病例100例,均合并Ⅰ型或Ⅱ型糖尿病,随机分成两组,腹腔镜阑尾切除组(LA)50例,开腹阑尾切除组(OA)50例,分析两组病人的临床资料,比较两组病人的手术时间、排气时间、术后第1 d疼痛评分、术后第1d血液白细胞水平、术后切口感染发生率、住院时间、及住院总费用。结果 (1)腹腔镜阑尾切除术组的手术时间与住院总费用与开腹阑尾切除组相比,差异无统计学意义(P>0.05);(2)腹腔镜阑尾切除组与开腹阑尾切除组相比,排气时间、住院时间明显缩短,差异有统计学意义(P<0.05);(3)术后第1d疼痛评分、术后第1d血液白细胞水平、术后切口感染发生率都明显降低,差异有统计学意义(P<0.05)。结论 腹腔镜阑尾切除术可以加快患者的恢复进程,减少病人的痛苦,降低阑尾炎合并糖尿病患者的术后并发症。

Objective To analyze and compare the clinical efficacy of laparoscopy and laparotomy in the treatment of appendicitis complicated with diabetes.Methods A total of 100 patients with acute appendicitis in the First Affiliated Hospital of Henan University of Science and Technology,all with type I or type II diabetes,were randomly divided into two groups,50 cases in the laparoscopic appendectomy group(LA) and 50 cases in the open appendectomy group(OA).,analyze the clinical data of the two groups of patients,and compare the operation time,exhaust time,pain score on the first postoperative day,blood leukocyte level on the first postoperative day,incidence of postoperative wound infection,hospitalization time,and hospitalization of the two groups of patients.Total cost.Results(1) There was no significant difference in the operation time and total hospitalization cost between the laparoscopic appendectomy group and the open appendectomy group(P>0.05);(2) Compared with the laparoscopic appendectomy group and the open appendectomy group,the difference was not statistically significant(P>0.05).The exhaust time and hospitalization time were significantly shortened,and the difference was statistically significant(P<0.05).(3) The pain score on the first day after the operation,the blood leukocyte level on the first day after the operation,and the incidence of postoperative incision infection were significantly reduced,and the differences were as follows:Statistical significance(P<0.05).Conclusions Laparoscopic appendectomy can speed up the recovery process of patients,reduce the pain of patients,and reduce postoperative complications in patients with appendicitis complicated with diabetes mellitus,which can be promoted in clinical practice.
