威高3D打印牙支持式种植导板的临床精度分析

摘要 目的:本研究对国产威高3D打印牙支持式种植导板进行精确度评价。方法:选取2019年1月至2020年12月本院收治的牙列缺损患者31例,在威高3D打印种植导板引导下植入种植体63枚。将植入手术前利用3 Shape Implant Studio软件设计的种植体植入数据与手术后采集的CBCT数据导入软件,选取上下颌骨的特殊解剖标记和缺牙区近远中健康邻牙的牙冠外形作为参照,配准后得到拟合图像,对术前设计的种植体位置和种植体的实际植入位置之间的偏差进行测量分析,得到种植体颈部中点及根端中点的距离偏差值和角度偏差值。结果:本研究经数据统计,得出种植体颈部及根方的近远中向、颊舌向及垂直向距离偏差的平均值均小于1 mm,近远中和颊舌向角度偏差的平均值均小于3o。与第三届欧洲骨结合大会中通过文献回顾统计出的数字化导板的平均偏差(肩台处与根尖处偏差分别为1.09 mm和1.28 mm,角度偏差为3.9o)相比,符合临床种植精确度要求。结论:威高数字化种植导板设计系统可在一定程度上简化复杂种植手术难度,提高手术精确度,临床可合理选择。 Objective:To investigate the accuracy and clinical effect of WEGO digital implant guides based on cone beam computed tomography(CBCT).Methods:31 dentition defect patients attended in our hospital from January 2019 to December 2020 were enrolled,who received tooth implantation surgery using WEGO digital implant guides.The pre-operative CBCT was taken,and operation plan was designed with 3 Shape Implant Studio,and the implants were placed assisted by digital guides produced by rapid prototyping technology,then post-operative CBCT was taken.Pre-and post-operative CBCT data was matched and integrated using 3 Shape Implant Studio,the position of the planned implants was compared to the actual position of the implants,the distance deviations and angle deviations of the implants were analyzed.Results:The average distance deviations of the implants at the neck and root level were both controlled within 1 mm and the average angular deviation at both level was less than 3o.All those deviations met the requirements for clinical planting accuracy according to European Association Organization.Conclusion:Tooth implantation using WEGO digital implant guides designed by CBCT has the potential to simplify complicated cases and improve surgical accuracy of dental implantation,so the clinical application can be selected based on the patients’condition.
