成人支气管哮喘与代谢及炎症因子的相关性研究 被引量:1

Correlation study between metabolism and inflammatory factors in adult with bronchial asthma

摘要 目的探讨代谢及炎症因子在成人支气管哮喘急性发作期中变化情况,分析不同哮喘表型中血脂、炎症因子及吸入物过筛试验(Phadiatop)之间的相关性。方法收集哮喘急性发作期病人134例进行Phadiatop、代谢及炎症因子检测,同期120例健康体检成人为对照组,检测代谢及炎症因子。结果哮喘组与对照组之间比较发现,总胆固醇(TC)、甘油三酯(TG)、极低密度脂蛋白(VLDL)、尿酸(UA)、同型半胱氨酸(HCY)和血糖(GLU)差异有统计学意义(P<0.05)。在炎症因子方面,两组之间白细胞(WBC)、中性粒细胞绝对值(NEUT)、嗜酸性粒细胞绝对值(EOS)及其百分比存在差异(P<0.05)。过敏性哮喘与非过敏性哮喘中TG、VLDL、白介素6(IL-6)和C-反应蛋白(CRP)水平有明显差异(P<0.05),相关性分析显示,IL-6与CRP、VLDL与EOS、IL-6与Phadiatop和NEUT与EOS均有相关性。结论支气管哮喘急性发作期病人与健康成人代谢及炎症因子间存在差异;与非过敏性哮喘相比,TG、VLDL、IL-6和CRP在过敏性哮喘更低,IL-6、CRP、VLDL、EOS、Phadiatop可能是不同哮喘表型的重要临床特征。 Objective To investigate the changes of metabolic and inflammatory factors in the acute exacerbation of adult with bronchial asthma,and to explore the association of blood lipids,inflammatory factors and Phadiatop(Inhalation Screening Test)in different asthma phenotypes.Methods A total of 134 patients with acute exacerbation of asthma were collected.Phadiatop,metabolism and inflammatory factors were detected.120 healthy adults were taken as the control group and their metabolism and inflammatory factors were measured.Results The comparison between the adult asthma group and the healthy control group showed that there were statistically significant differences in total cholesterol(TC),triglycerides(TG),very low density lipoprotein(VLDL),uric acid(UA),homocysteine(HCY)and blood sugar(GLU)(P<0.05).There were statistically significant differences in white blood cells(WBC),neutrophils(NEUT),eosinophils(EOS)and the percentage of eosinophils(EOS%)between the asthma group and the control group(P<0.05).The comparison between allergic asthma and non-allergic asthma indicated that the levels of TG,VLDL,interleukin 6(IL-6)and C-reactive protein(CRP)were significantly different(P<0.05).Correlation analysis in different phenotypes of asthma showed that IL-6 was correlated with CRP,VLDL was correlated with EOS,IL-6 was correlated with Phadiatop,and NEUT was correlated with EOS.Conclusion There are differences in the metabolism and inflammatory factors between patients with acute bronchial asthma and healthy adults.Compared with non-allergic asthma,TG,VLDL,IL-6 and CRP are lower in allergic asthma.IL-6,CRP,VLDL,EOS and Phadiatop may be the important clinical features of different asthma phenotypes.
