糖尿病合并脑卒中患者实施综合护理干预对肢体功能和心理状态的影响

Effects of Comprehensive Nursing Intervention on Limb Function and Psychological State in Patients with Diabetes and Stroke

目的探讨综合护理干预对糖尿病合并脑卒中患者肢体功能和心理状态的影响。方法选取2019年4月—2020年4月福州市晋安区医院收治的糖尿病合并脑卒中患者94例,随机分为两组,各47例。对照组实施常规护理,观察组接受综合护理,分别于患者护理前(T_(0))及护理后(T_(1))对比两组肢体功能[简式Fugl-Meyer评定法(FMA)]、心理状态[抑郁自评量表(SDS)、焦虑自评量表(SAS)]、生存质量[世界卫生组织生存质量测定量表(WHOQOL-BREF)]、护理满意度[纽卡斯尔护理服务满意度量表(NSNS)]。结果T_(1)时两组FMA评分均升高,且观察组高于对照组,差异有统计学意义(P<0.05);T_(1)时两组SDS、SAS评分均下降,且观察组低于对照组,差异有统计学意义(P<0.05);T_(1)时两组躯体健康、心理状态、社会关系和环境因素评分均升高,且观察组高于对照组,差异有统计学意义(P<0.05);观察组护理满意度为95.74%,高于对照组的80.85%,差异有统计学意义(P<0.05)。结论对糖尿病合并脑卒中患者以综合护理进行干预,可提高患者肢体功能,降低患者焦虑、抑郁情绪,提高患者生存质量和护理满意度。

Objective To explore the effect of comprehensive nursing intervention on limb function and psychological state of diabetic patients with stroke.Methods From April 2019 to April 2020,94 patients with diabetes complicated with stroke were randomly divided into two groups,47 cases in each group.The control group received routine nursing,and the observation group received comprehensive nursing.Before nursing(T_(0))and after nursing(T_(1)),the limb function[abbreviated Fugl-Meyer Assessment(FMA)],psychological state[Self-rating Depression Scale(SDS)and Self-rating Anxiety Scale were compared between the two groups(SAS)],quality of life[World Health Organization Quality of Life Measurement Scale(WHOQOL-BREF)],nursing satisfaction[Newcastle Nursing Service Satisfaction Scale(NSNS)].Results At T_(1),the FMA scores of the two groups increased,and the observation group was higher than the control group,the difference was statistically significant(P<0.05);at T_(1),the SDS and SAS scores of the two groups decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);at T_(1),the scores of physical health,psychological state,social relations and environmental factors in the two groups were increased,and the observation group was higher than the control group,the difference was statistically significant(P<0.05);the nursing satisfaction of the observation group was 95.74%,which was higher than 80.85%of the control group,and the difference was statistically significant(P<0.05).Conclusion Intervention with comprehensive nursing for diabetic patients with stroke can improve their limb function and reduce their anxiety and depression,improve patient quality of life and nursing satisfaction.
