慢性心力衰竭患者居家休养期间社会支持与健康管理需求的调查

Investigation on social support and health management needs of chronic heart failure patients during home recuperation

目的:调查慢性心力衰竭(CHF)患者居家休养期间社会支持与健康管理需求。方法:选取2019年5月-2021年5月于我院接受诊治后出院的102例CHF患者,采用问卷调查法,包括一般资料、社会支持评定量表(SSRS)、心力衰竭特异性健康素养评分及健康管理需求评估内容,整理调查结果。结果:CHF患者居家休养期间开展健康管理需求共87例(85.29%);CHF患者对健康管理内容需求以疾病知识占比最高为24.51%,出院后转介机构以家庭占比最高为90.20%,居家休养期间可利用人员以子女占比最高为62.74%,对健康管理方式需求以电话占比最高为65.59%。结论:由于我国多数地区对CHF患者出院后无法提供完善社会支持服务,使得多数患者选择居家休养,需对患者主要照顾人群开展健康教育,并采取适宜随访方式,以促进患者社会功能恢复,并改善家庭功能。

Objective:To investigate the needs of social support and health management in patients with chronic heart failure(CHF)during home recuperation.Methods:A total of 102 patients with CHF who were discharged from our hospital from May 2019 to May 2021 were selected.Questionnaire survey was used,including general information,social support rating scale(SSRS),heart failure-specific health literacy score and Health management needs assessment content,sorting out survey results.Results:There were 87 cases(85.29%)of CHF patients who needed to carry out health management during home recuperation;CHF patients’demand for health management content was disease knowledge accounted for the highest proportion of 24.51%;During the recuperation period,the highest proportion of available personnel is children,62.74%,and the highest proportion of telephone calls for health management methods is 65.59%.Conclusion:Since most areas in my country cannot provide comprehensive social support services for CHF patients after discharge,most patients choose to recuperate at home.Health education should be carried out for the main care groups of patients,and appropriate follow-up methods should be adopted to promote the recovery of patients’social functions and improve family members.Function.
