术中预防性护理结合中医疼痛护理预防老年髋部骨折术后谵妄的分析

Analysis of intraoperative preventive nursing combined with traditional Chinese medicine pain nursing to prevent postoperative delirium in elderly hip fractures

目的:研究术中预防性护理联合中医疼痛护理预防老年髋部骨折术后谵妄(POD)的效果。方法:选取2020年1月-2022年1月收治的78例老年髋部骨折患者,分组按随机数字表法,对照组(39例)行常规护理,试验组(39例)行术中预防性护理联合中医疼痛护理,对比两组生活质量、术后谵妄发生率、疼痛程度及心理状态。结果:试验组护理后SAS评分、SDS评分低于对照组(P<0.05);试验组术后1d、术后3d及术后5dVAS评分低于对照组(P<0.05);试验组术后谵妄发生率低于对照组(P<0.05);试验组SF-36各项评分高于对照组(P<0.05)。结论:术中预防性护理联合中医疼痛护理应用于老年髋部骨折手术患者,可有效预防POD,有利于疼痛减轻,并促进心理状态改善,进而提高患者生活质量。

Objective:To study the effect of intraoperative preventive nursing combined with traditional Chinese medicine pain nursing in preventing postoperative delirium(POD)in elderly patients with hip fracture.Methods:A total of 78 elderly patients with hip fracture who were treated from January 2020 to January 2022 were selected and divided into groups according to the random number table method.The control group(39 cases)received routine care,and the experimental group(39 cases)received intraoperative preventive care.Combined with traditional Chinese medicine pain care,the quality of life,the incidence of postoperative delirium,the degree of pain and the psychological state were compared between the two groups.Results:The SAS score and SDS score of the experimental group after nursing were lower than those of the control group(P<0.05);the VAS score of the experimental group was lower than that of the control group at 1d,3d and 5d after operation(P<0.05);the incidence of postoperative delirium in the experimental group The rate of SF-36 in the experimental group was higher than that in the control group(P<0.05).Conclusion:Intraoperative preventive nursing combined with traditional Chinese medicine pain nursing in elderly patients with hip fracture surgery can effectively prevent POD,help reduce pain,and promote the improvement of psychological state,thereby improving the quality of life of patients.
