细胞免疫状况和宫颈癌患者手术后留置尿管感染的关系研究

Relationship between cellular immune status and indwelling urinary tract infection in patients with cervical cancer after surgery

目的:探索细胞免疫状况和宫颈癌患者手术后留置尿管感染的关系。方法:选取2018年1月至2019年8月于某院接受治疗的60例宫颈癌患者为研究对象,术前对两组患者进行T淋巴细胞水平检查,其中30例CD3+水平<50%的患者为观察组,30例CD3+水平>50%的患者为对照组。比较两组患者手术状况、血清炎性因子以及感染情况。结果:两组患者的出血量、手术时间无显著差异,数据比较不具有统计学意义(P>0.05)。观察组的住院时间显著高于对照组,数据比较具有统计学意义(P<0.05);观察组术前、术后1 d以及术后3 d的肿瘤坏死因子α、白细胞介素-1β、白细胞介素-10以及白细胞介素-8水平均显著高于对照组,数据比较具有统计学意义(P<0.05);观察组留置尿管感染例数及感染率显著高于对照组,数据比较具有统计学意义(P<0.05)。结论:术前细胞免疫状态较差的患者,术后其体内含有较高水平的促炎性细胞因子,显著增加了患者发生留置尿管感染等并发症的可能性,使患者住院时间延长,给患者及其家属带来了沉重的心理和经济压力。

Objective:To explorethe relationship between cellular immune status and indwelling urinary tract infection in patients with cervical cancer after surgery.Methods:60 cases of cervical cancer patients treated in our hospital from January 2018 to August 2019 were selected as the observation group Before the operation,Tlymphocyte levels were examined.30 cases of cervical cancer patients who had CD3+level<50%as the observation goup.Patients who had CD3+level>50%as the control group.Compare the surgical status,serum inflammatory factors and infection of the two groups of patients.Results:There was no significant difference in bleeding volume and operation time between the two groups of patients,and the data comparison was not statistically significant(P>0.05).The time of hospital stay in the observation group was significantly longer than that in the control group.The data comparison was statistically significant(P<0.05);The levels of tumor necrosis factorα,interleukin-1β,interleukin-10 and interleukin-8 in the observation group were significantly higher than those in the control group before surgery,1 day after surgery,and 3 days after surgery.The data comparison was statistically significant(P<0.05);There were indwelling urinary tube infection in the observation group,with an infection rate,which was significantly higher than that in the control group.The infection rate was 6.67%,and the data were statistically significant(P<0.05).Conclusion:Patients with poor cellular immune status before surgery have higher levels of pro-inflammatory cytokines in the body after surgery,which significantly increases the possibility of complications such as indwelling urinary tract infections and prolongs the time of hospital stay,have brought heavy psychological and economic pressure for patients and Their familes.
