基于DRG和综合指数法的住院患者医疗服务绩效评价

Performance Evaluation of Inpatient Medical Services Based on DRG and Composite Index Method

目的 通过对医疗服务产出、医生的技术水平、医疗资源的消耗程度及医疗质量等进行综合评价,分析样本医院的医疗服务情况差异,为提升医疗服务绩效和医疗水平提供参考。方法 以患者较多、病种复杂、医疗难度较大的普外科为例,收集2020年各病区住院患者相关数据,采用疾病诊断相关分组(DRG)指标结合综合指数法,从服务能力、效率、安全等维度对普外科6个病区绩效进行评价。结果 普外科6个病区医疗服务水平存在差异,在医疗服务能力、效率维度各有优劣。结论 DRG结合综合指数法能有效应用于住院患者医疗服务绩效评价,为促进病区提升医疗服务能力、改善医疗服务效率、优化安全质量考核指标、实现整体均衡发展提供建议与策略。

Objective To analyze the differences of medical service situation in sample hospitals through comprehensive evaluation of medical service output, doctors’ technical level and the consumption degree of medical resources and medical quality,and to provide references for improving medical service performance and medical standards. Method Taking the general surgery department with more patients,complex diseases and medical difficulties as an example,data related to inpatients in each ward in 2020 were collected,and DRG indicators combined with the comprehensive index method were used to evaluate the performance of six wards of general surgery from the dimensions of service capacity, efficiency and safety. Result There were differences in the level of medical services among 6 wards of general surgery,each with its own advantages and disadvantages in terms of medical service capacity and efficiency dimensions. Conclusion DRG combined with the comprehensive index method can be effectively used for inpatient medical service performance evaluation,providing suggestions and strategies to promote wards to enhance medical service capacity,improve medical service efficiency,optimize safety and quality assessment indexes,and overall balanced development.
