温针灸与单纯针刺治疗老年膝骨关节炎临床观察

Clinical Observation on Warming Needle Moxibustion and Simple Acupuncture in the Treatment of Senile Knee Osteoarthritis

目的 探讨温针灸与单纯针刺治疗老年膝骨关节炎(Knee osteoarthritis,KOA)的临床效果。方法 将严格筛选的84例老年KOA患者按随机数字表法分成温针灸组和针刺组,2组均42例。取穴为梁丘、阳陵泉、血海、内膝眼、外膝眼、气海和关元,均持续治疗2个疗程并比较2组治疗效果。结果 2组治疗前和治疗2、4周Lequesne-MG评分均依次明显下降(P <0.05),温针灸组治疗2、4周Lequesne-MG评分明显低于针刺组(P <0.05)。2组治疗4周WOMAC量表各维度和总分均较治疗前明显下降(P <0.05),温针灸组治疗4周WOMAC量表各维度和总分均低于针刺组(P <0.05)。温针灸组临床总有效率显著高于针刺组(P <0.05)。结论 温针灸和单纯针刺疗法对老年KOA患者均有积极效果,但温针灸在缓解KOA症状和改善膝关节功能方面更具优势,临床疗效显著。

Objective To explore the clinical effect of warming needle moxibustion and simple acupuncture in the treatment of senile knee osteoarthritis(KOA). Methods According to the grouping method of randomized controlled trial,84 elderly patients with KOA were divided into warm acupuncture group and acupuncture group. There were 42 cases in both groups. The points were Liangqiu(ST34),Yanglingquan(GB34),Xuehai(SP10),Neixiyan(EX-LE4),Waixiyan,Qihai(RN6) and Guanyuan(RN4). They were treated for two courses,and the therapeutic effects of the two groups were compared. Results The Lequesne mg scores of the two groups decreased significantly before treatment and at 2 and 4 weeks of treatment(P < 0. 05). The Lequesne-MG scores of the warm acupuncture group were significantly lower than those of the acupuncture group at 2 and 4 weeks of treatment(P < 0. 05). After 4 weeks of treatment,the dimensions and total scores of WOMAC scale in the two groups were significantly lower than those before treatment(P < 0. 05). The dimensions and total scores of WOMAC scale in warm acupuncture group were lower than those in acupuncture group(P < 0. 05). The total clinical effective rate of warm acupuncture group was significantly higher than that of acupuncture group(P < 0. 05). Conclusion Warming needle moxibustion and simple acupuncture have positive effects on elderly patients with KOA,but warming needle moxibustion has more advantages in alleviating KOA symptoms and improving knee function.
