复发性肩关节前脱位影像特征分析:与关节镜对照

摘要 目的 以关节镜检查为金标准,探讨MRI、多层螺旋CT(multi slice spiral CT,MSCT)在复发性肩关节前脱位的诊断价值,以及关节脱位次数、关节囊松弛对Hill-Sachs损伤分度的影响。方法 回顾性分析38例复发性肩关节前脱位患者关节镜、磁共振及MSCT检查结果,分别记录Bankart损伤、Hill-Sachs损伤、关节囊松弛例数及临床脱位次数,分析影像学特征与关节镜的一致性。结果 (1)关节镜示Bankart损伤38例、骨性Bankart损伤16例、Hill-Sachs损伤38例,关节囊松弛19例;CT示骨性Bankart损伤16例、Hill-Sachs损伤38例;MRI示Bankart损伤38例、骨性Bankart损伤14例、Hill-Sachs损伤38例。(2)关节脱位<5次与≥5次患者的Hill-Sachs损伤分级比较,差异无统计学意义(P=0.917);关节囊松弛与关节囊无松弛患者的Hill-Sachs损伤分级比较,差异无统计学意义(P=0.355)。结论 Bankart损伤的MRI诊断准确率与关节镜相近,CT仅能显示骨性Bankart损伤,而Hill-Sachs损伤的CT、MRI准确率与关节镜一致;Hill-Sachs损伤分度与关节脱位次数、关节囊松弛没有相关性。
