益肾固龈汤联合牙周刮治对重度牙周病患者的影响

Discussion on the Clinical Efficacy and Safety of Yishen Guyin Decoction Combined with Periodontal Curettage in Patients with Severe Periodontal Disease

目的 探讨益肾固龈汤联合牙周刮治对重度牙周病患者的临床疗效及安全性。方法 选取2018年2月—2021年2月确诊的重度牙周病患者78例作为观察对象,按照随机数字表法分2组,其中对照组39例,采用牙周刮治治疗;观察组39例,予以益肾固龈汤联合牙周刮治治疗。观察牙周出血指数分级、菌斑指数分级及牙周探诊深度(PD)情况;分别于治疗前后测定IL-6、CRP及IFN-α水平变化;评估疼痛程度及临床疗效。结果 治疗2个月后,观察组BI 0~1级及PLI 0~1患者所占比例均较对照组高(P<0.05);观察组PD较对照组低。治疗后,观察组IL-6、CRP及IFN-α水平及VAS评分均低于对照组(P<0.05)。治疗后,观察组临床总有效率高于对照组(P<0.05)。此外,2组均未见其他严重不良情况。结论 益肾固龈汤联合牙周刮治用于重度牙周病患者临床疗效显著,不仅能够改善牙周出血指数、菌斑指数及牙周探诊深度还能降低炎症水平,缓解疼痛症状,且安全性高。

Objective To explore the clinical efficacy and safety of Yishen Guyin decoction combined with periodontal curettage in patients with severe periodontal disease. Methods 78 patients with severe periodontal disease diagnosed in our hospital from February 2018 to February 2021 were selected as observation objects,and were divided into two groups according to the random number table method. Among them,39 patients in the control group were treated with periodontal curettage. 39 cases in the observation group were treated with Yishen Guyin decoction combined with periodontal curettage. The periodontal bleeding index grading,plaque index grading and periodontal probing depth;measure the changes of IL-6,CRP and IFN-α levels before and after treatment were observed. The degree of pain and clinical efficacy was evaluated. Results After 2 months of treatment,the proportions of patients with BI 0 ~ 1 and PLI 0 ~ 1 in the observation group respectively,which were higher than the control group( P< 0. 05);observation;The PD of the group was lower than that of the control group. After treatment,the levels of IL-6,CRP and IFN-α and VAS scores in the observation group were lower than those in the control group( P< 0. 05). After treatment,the total clinical effective rate of the observation group was higher than that of the control group( P<0. 05). In addition,there were no other serious adverse events in the two groups. Conclusion Yishen Gugingan Decoction combined with periodontal curettage has a significant clinical effect in the treatment of patients with severe periodontal disease. It can not only improve periodontal bleeding index,plaque index and depth of periodontal exploration,but also reduce inflammation and relieve pain symptoms,And high security.
