摘要
目的探讨卵巢癌癌因性疲乏(cancer-related fatigue,CRF)与卵巢功能丧失的相关性。方法回顾性分析2015年1月至2020年1月苏州大学附属第一医院收治的211例卵巢癌幸存者资料。采用癌症疲劳量表(cancer fatigue scale,CFS)测量癌因性疲乏的程度并评估调查。采用单线性回归和多元线性回归的方法对研究数据进行统计。结果211例卵巢癌幸存者中,206例患者报告有疲乏症状。在初始治疗前仍有月经患者的CFS评分明显高于初始治疗前已经绝经的患者,差异有统计学意义(t=2.650,P=0.009)。CFS评分在接受治疗后的前2年最高,并随着时间的推移逐渐下降。有睡眠障碍的患者更容易疲乏(P=0.002),而且血红蛋白越低,CFS评分越高(r=-0.156,P=0.023)。CRF与化疗周期数、化疗方案及卵巢癌的病理类型之间无显著相关性(均P>0.05)。结论CRF在卵巢癌患者中普遍存在,激素水平突然下降会导致CRF发生率升高,说明CRF可能与性激素的缺乏有关。为了改善CRF,可考虑在治疗方案中适当补充激素。
Objective To investigate the association between cancer-related fatigue and the loss of ovarian function in patients with ovarian cancer.Methods Retrospective analysis was performed in 211 survivors of ovarian carcinoma treated in the First Affiliated Hospital of Soochow University between January 2015 and January 2020.Fatigue was measured with the Cancer Fatigue Scale(CFS)and was assessed.Single and multiple linear regression were used to determine statistical differences.Results Fatigue was reported in 206 of all completed questionnaires.Patients who had a menstrual period prior to treatment had a higher fatigue score than those who had no menstruation(t=2.650,P=0.009).The CRF score was highest during the first two years after treatment and gradually decreased over time.Patients with sleep disorders became fatigued more easily(P=0.002).We identified a negative correlation between hemoglobin and CFS(r=-0.156,P=0.023).There were no significant correlations between CRF and the number of chemotherapy cycles,the type of chemotherapy regimen,or the pathological subtype of ovarian cancer(P>0.05).Conclusions CRF is common in ovarian cancer patients and the sudden decrease of horme level leads to increase in CRF incidence.The fatigue experienced by patients with ovarian cancer may be related to the deficiency of sex hormones.It may be good to add such hormones to the treatment regimen in order to improve CRF.
