白介素-34和白介素-35与内服外治法治疗早期强直性脊柱炎疗效相关性研究

Study on the correlation between interleukin-34 and interleukin-35 and the efficacy of oral and external therapy in the treatment of early ankylosing spondylitis

目的:探讨白介素-34(IL-34)、白介素-35(IL-35)与内服外治法治疗早期强直性脊柱炎疗效的相关性。方法:选取2019年9月—2021年9月临床诊断为早期强直性脊柱炎患者50例为研究对象,给予甘草附子汤联合长蛇灸治疗,并检测患者治疗前后外周血IL-34和IL-35水平,评定治疗前后中医证候评分、视觉模拟评分(VAS)、生活质量评分和临床疗效,并分析外周血IL-34和IL-35水平与疗效的相关性。结果:与治疗前比较,治疗后患者的IL-34水平显著降低,IL-35水平显著升高(P<0.05);与治疗前比较,治疗后患者中医证候评分、VAS疼痛评分降低,生活质量评分升高(P<0.05);50例早期强直性脊柱炎患者中,治愈22例(44.00%),显效16例(32.00%),有效10例(20.00%),无效2例(4.00%),有效率96.00%。经相关性分析显示,疗效与IL-34水平呈负相关(r=-0.218,P=0.002),与IL-35水平呈正相关(r=0.346,P=0.007)。患者在治疗期间未发生明显不良反应。结论:早期强直性脊柱炎患者疗效与IL-34和IL-35水平有关,内服外治疗法疗效较好。

Objective:To explore the correlation between interleukin-34(IL-34),interleukin-35(IL-35)and the efficacy of oral and external therapy in the treatment of early ankylosing spondylitis.Methods:From September 2019 to September 2021,50 patients with early ankylosing spondylitis clinically diagnosed in hospital were selected as the research subjects,and the patients were treated with gancao fuzi decoction combined with long snake moxibustion.The levels of IL-34 and IL-35 in peripheral blood were detected before and after treatment,and the scores of TCM syndromes,visual analogue scale(VAS),total quality of life and clinical efficacy were evaluated before and after treatment.The correlation between the levels of IL-34 and IL-35 in peripheral blood and curative effect was analyzed.Results:Compared with before treatment,the level of IL-34 in patients after treatment was significantly decreased,and the level of IL-35 was significantly increased(P<0.05).Compared with before treatment,the TCM syndrome score and VAS pain score of the patients decreased,and the quality of life improved after treatment(P<0.05)After treatment,22 cases(44.00%)of 50 patients with early ankylosing spondylitis were cured,16 cases(32.00%)were markedly effective,10 cases were effective(20.00%),and 2 cases(4.00%)were ineffective.The effective rate was as high as 96.00%,the correlation analysis showed that the efficacy was negatively correlated with the level of IL-34(r=-0.218,P=0.002),and positively correlated with the level of IL-35(r=0.346,P=0.007).The patient had no obvious adverse reactions during the treatment.Conclusion:The curative effect of patients with early ankylosing spondylitis is related to the levels of IL-34 and IL-35,and the curative effect of internal and external therapy is good and obvious.
