139名肠造口患儿主要照顾者家庭坚韧性现状及影响因素分析

Current Status of Family Resilience of Primary Caregivers of Children with Enterostomy and Its Influencing Factors: A 139-case Study

目的调查肠造口患儿主要照顾者家庭坚韧性现状,分析其影响因素,为寻求提升家庭坚韧性的干预措施提供参考。方法采用便利抽样法,选取<14岁的肠造口患儿主要照顾者139名为研究对象,采用一般资料调查表、家庭功能量表-总的功能分量表、肠造口患儿家庭主要照顾者照护能力测评量表和家庭坚韧性表进行调查。采用多重线性回归分析其家庭坚韧性的影响因素。结果本组肠造口患儿主要照顾者家庭坚韧性总分为(49.68±8.66)分,与家庭功能-总的功能呈负相关(r=-0.780,P<0.01),与照护能力呈正相关(r=0.759,P<0.01);多重线性回归分析结果显示,文化程度、家庭功能及主要照顾者照护能力3个因子可解释肠造口患儿主要照顾者家庭坚韧性总变异的71.1%。结论本组肠造口患儿主要照顾者家庭坚韧性处于中等水平,提示医护人员通过评估肠造口患儿主要照顾者的家庭坚韧性水平来辨识家庭的弱势和资源,关注主要照顾者的情感心理健康,运用以家庭为中心的护理实践来促进肠造口患儿身体康复,为寻求提升家庭坚韧性的干预措施提供参考,以更好地为身处逆境中的家庭提供指导。

Objective To investigate the current status of family resilience of the primary caregivers of children with enterostomy,to analyze its influencing factors,and to provide reference for intervention to improve family resilience.Methods A total of 139 primary caregivers of children with enterostomy who were younger than 14 years old were selected by convenient sampling and they were surveyed by using general information questionnaire,Family Assessment Device-General Functioning Scale,Care Ability Evaluation Scale of Primary Caregivers of Children with Enterostomy and Family Hardiness Index.Multiple linear regression was used to analyze the influencing factors of family resilience.Results The total score of family resilience of the primary caregivers of children with enterostomy was 49.68±8.66,which was negatively correlated with family function-total function(r=-0.780,P<0.01),and positively with care ability(r=0.759,P<0.01).Multiple linear regression analysis showed that three factors of education background,family function and care ability of primary caregivers could explain 71.1%of the total variation.Conclusion Family resilience of the main caregivers of children with enterostomy in this group is at a medium level,suggesting that medical staff can identify the weaknesses and resources of the family by evaluating the family resilience of the main caregivers.Emotional and mental health of the main caregivers,and family-centered nursing practice are conducive to promoting physical rehabilitation of children with enterostomy,so as to provide reference for intervention measures to improve family resilience and guidance for families in adversity.
