四川省5所三级甲等医院急诊科医务人员心理一致感、职业应激对睡眠质量的影响

Impact of psychological consistency and occupational stress on sleep quality among medical staff in emergency department of five grade-A tertiary hospitals in Sichuan Province

目的 了解四川省三级甲等医院急诊科医务人员心理一致感、职业应激与睡眠质量现状及其相关性,探讨心理一致感在职业应激与睡眠质量间的中介作用。方法 运用方便抽样法于2021年5-6月选取四川省5所三级甲等医院急诊科医务人员为调查对象,采用心理一致感量表、职业应激量表、睡眠质量量表对323名医护人员进行调查,并对数据进行分析。结果 323名医务人员心理一致感得分为(56.89±10.29)分,职业应激得分为(601.82±149.01)分,睡眠质量得分为(9.00±3.49)分。心理一致感总分及各维度得分与睡眠质量总分及各维度得分均呈负相关(r=-0.283~-0.848,均P<0.05),职业应激总分及各维度得分与睡眠质量总分及各维度得分均呈正相关(r=0.356~0.933,均P<0.05),心理一致感总分及各维度得分与职业应激总分及各维度得分均呈负相关(r=-0.409~-0.772,均P<0.05)。心理一致感在急诊科医务人员职业应激与睡眠质量之间有正向的中介作用,中介效应为0.221,中介效应占总效应的(0.221/0.425)×100%=52.00%。结论 四川省5所三级甲等医院急诊科医务人员睡眠质量不佳,心理一致感对睡眠质量具有直接负向作用,心理一致感在职业应激与睡眠质量中存在直接正向作用。医务人员心理一致感,降低职业应激,能改善医务人员睡眠质量,促进其身心健康。

Objective To investigate the current situation and correlation of psychological consistency,occupational stress and sleep quality among emergency medical staff in grade-A tertiary hospitals of Sichuan Province,explore the mediating role of psychological consistency between occupational stress and sleep quality.Methods The convenience sampling method was adapted to select the medical staff in the emergency department of five grade-A tertiary hospitals in Sichuan Province from May to June2021 as the research object. A total of 323 medical workers were investigated with the psychological consistency scale,occupational stress scale and sleep quality scale,and the data were analyzed. Results The score of psychological consistency,occupational stress and sleep quality in 323 medical workers was(56.89±10.29),(601.82±149.01) and(9.00±3.49)points,respectively. The total score of psychological consistency and the scores of each dimension were negatively correlated with the total score of sleep quality and the scores of each dimension(r=-0.283--0.848,all P<0.05). The total score of occupational stress and the score of each dimension were positively correlated with the total score of sleep quality and the scores of all dimensions(r =0.356-0.933,all P <0.05). The total score of psychological consistency and the scores of each dimension were negatively correlated with the total score of occupational stress and the scores of each dimension(r =-0.409--0.772,all P <0.05). The psychological consistency had a positive mediating effect between occupational stress and sleep quality of medical staff in emergency department,and the mediating effect was 0.221,accounting for(0.221/0.425)×100%=52.00% of the total effect.Conclusion The sleep quality of medical staff in the emergency department of five grade-A tertiary hospitals in Sichuan Province is not good,the psychological consistency has a direct negative effect on sleep quality,and the psychological consistency has a direct positive effect on occupational stress and sleep quality. Medical staff’s psychological consistency and reducing occupational stress can improve their sleep quality and promote physical and mental health.
