肿瘤特异性生长因子、抗甲状腺球蛋白抗体与抗过氧化物酶抗体联合检测在鉴别诊断甲状腺结节的意义

Significance of Combined Detection of Tumor Specific Growth Factor,Anti Thyroglobulin Antibody and Anti-peroxi-dase Antibody in Differential Diagnosis of Thyroid Nodules

目的:探讨肿瘤特异性生长因子(TSGF)、抗甲状腺球蛋白抗体(Tg-Ab)、抗过氧化物酶抗体(Tpo-Ab)的血清学水平测定在甲状腺结节的良恶性鉴别诊断中的临床意义。方法:选取2018年1月—2019年12月在广东省农垦中心医院收治的132例甲状腺结节的患者作为研究对象,按术后病理诊断结果分为单纯良性结节甲状腺肿组(33例)、甲状腺癌组(47例)、甲状腺瘤组(52例),分别测定各组其术前TSGF、Tg-Ab、Tpo-Ab的血清水平及阳性检出率。并收集同期医院体检人群50例作为正常对照组。结果:甲状腺癌组的TSGF、Tg-Ab、Tpo-Ab的血清水平明显高于其他三组,差异有统计学意义(t=10.422,P<0.05),单纯良性结节甲状腺肿组、甲状腺瘤组、正常对照组的TSGF、Tg-Ab、Tpo-Ab的血清水平组间比较,差异无统计学意义(t=0.737,P>0.05)。甲状腺癌组的TSGF、Tg-Ab、Tpo-Ab阳性率明显高于正常对照组,差异有统计学意义(χ^(2)=30.415,P<0.05),甲状腺癌组的TSGF、Tg-Ab的阳性率与单纯良性结节甲状腺肿组、甲状腺瘤组两两比较,差异有统计学意义(χ^(2)=12.659,P<0.05),甲状腺癌组的Tpo-Ab阳性率与其他两组的组间比较,差异无统计学意义(χ^(2)=0.922,P>0.05)。TSGF、Tg-Ab、Tpo-Ab联合检测甲状腺癌组的阳性率明显高于其他三组,差异有统计学意义(χ^(2)=35.416,P<0.05)。结论:甲状腺癌的TSGF、Tg-Ab、Tpo-Ab的血清水平及阳性检出率高,联合检测TSGF、Tg-Ab、TpoAb在甲状腺结节的良恶性鉴别诊断中有着重要的临床意义。

Objective:To explore the clinical significance of serological levels of tumor-specific growth factor(TSGF),anti-thyroglobulin antibody(Tg-Ab)and anti-peroxidase antibody(Tpo-Ab)in the differential diagnosis of benign and malignant thyroid nodules.Methods:A total of 132 patients with thyroid nodules admitted to the hospital from January 2018 to December 2019 were selected and divided into simple benign nodular goiter group(33 cases),thyroid cancer group(47 cases),and thyroid nodules tumor group(52 cases)according to postoperative pathological diagnosis.The serum levels and positive detection rates of TSGF,Tg-Ab and Tpo-Ab in each group were measured before operation.And 50 cases of physical examination population in the hospital during the same period were collected as the normal control group.Results:The serum levels of TSGF,Tg-Ab and Tpo-Ab in the thyroid cancer group were significantly higher than those in the other three groups,and the difference was statistically significant(t=10.422,P<0.05).There was no statistically significant difference in the serum levels of TSGF,Tg-Ab and Tpo-Ab between the simple benign nodular goiter group,thyroid tumor group and normal control group(t=0.737,P>0.05).The positive rates of TSGF,Tg-Ab and Tpo-Ab in the thyroid cancer group were significantly higher than those in the normal control group,and the difference was statistically significant(χ2=30.415,P<0.05).The positive rates of TSGF and Tg-Ab in the thyroid cancer group were significantly different from those in the simple benign nodular goiter group and thyroid tumor group(χ2=12.659,P<0.05).There was no statistically significant difference in the positive rate of Tpo-Ab between the thyroid cancer group and the other two groups(χ2=0.922,P>0.05).The positive rate of combined detection of TSGF,Tg-Ab and Tpo-Ab in thyroid cancer group was significantly higher than that in the other three groups,and the difference was statistically significant(χ2=35.416,P<0.05).Conclusion:The serum levels and positive detection rate of TSGF,Tg-Ab and Tpo-Ab in thyroid cancer are high,and the combined detection of TSGF,Tg-Ab and Tpo-Ab has important clinical significance in the differential diagnosis of benign and malignant thyroid nodules.
