仙灵骨葆胶囊联合盐酸氨基葡萄糖治疗膝骨关节炎临床研究

Clinical Study of Xianling Gubao Capsules Combined with Glucosamine Hydrochloride in the Treatment of Knee Osteoarthritis

目的探讨仙灵骨葆胶囊联合盐酸氨基葡萄糖治疗膝骨关节炎的临床疗效,以及对患者炎性因子水平的影响。方法选取河北省秦皇岛市中医医院2018年7月至2021年7月收治的膝骨关节炎患者109例,按随机抽签法分为观察组(55例)和对照组(54例)。对照组患者予盐酸氨基葡萄糖治疗,观察组患者在对照组治疗基础上加用仙灵骨葆胶囊,均治疗12周。结果观察组的总有效率为90.91%,显著高于对照组的74.07%(P<0.05)。治疗后,观察组患者的肿胀、疼痛、僵硬、跛行等中医证候积分均显著低于对照组(P<0.05);凝血功能指标凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)均显著长于对照组,纤维蛋白原(Fib)水平显著低于对照组(P<0.05);炎性因子单核细胞趋化因子-1(MCP-1)、抗瓜氨酸蛋白抗体(ACPA)、细胞间黏附分子-1(ICAM-1)水平均显著低于对照组(P<0.05)。观察组与对照组治疗期间药品不良反应发生率相当(14.55%比9.26%,P>0.05)。结论仙灵骨葆胶囊联合盐酸氨基葡萄糖治疗膝骨关节炎临床疗效较好,可有效改善患者的临床症状,其作用可能与改善凝血功能异常状态、降低机体的炎性因子水平有关,且治疗安全性良好。

Objective To investigate the clinical efficacy of Xianling Gubao Capsules combined with glucosamine hydrochloride inthe treatment of knee osteoarthritis,and its effects on the level of inflammatory factors in patients.Methods A total of 109patients with knee osteoarthritis admitted to the Qinhuangdao Traditional Chinese Medicine Hospital from July 2018 to July 2021were selected and randomly divided into the observation group(55 cases)and the control group(54 cases)according to the random draw method.The patients in the control group were treated with glucosamine hydrochloride,on this basis,the patients inthe observation group were treated with Xianling Gubao Capsules.Both groups were treated for 12 weeks.Results The totaleffective rate in the observation group was 90.91%,which was significantly higher than 74.07%in the control group(P<0.05).After treatment,the scores of traditional Chinese medicine(TCM)syndromes such as swelling,pain,stiffness and claudication in theobservation group were significantly lower than those in the control group(P<0.05).After treatment,the coagulation functionindexes such as prothrombin time(PT),thrombin time(TT)and activated partial thromboplastin time(APTT)in the observationgroup were significantly longer than those in the control group,and the level of fibrinogen(Fib)in the observation group wassignificantly lower than that in the control group(P<0.05).After treatment,the levels of inflammatory factors such as monocytechemoattractant protein-1(MCP-1),anti-citrullinated protein antibody(ACPA)and intercellular adhesion molecule-1(ICAM-1)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse drugreactions in the observation group was similar to that in the control group(14.55%vs.9.26%,P>0.05).Conclusion XianlingGubao Capsules combined with glucosamine hydrochloride is effective and safe in the treatment of knee osteoarthritis,which caneffectively improve the clinical symptoms of patients,and its effect may be related to improving the abnormal state of coagulationfunction and reducing the level of inflammatory factors in the body.
