先兆流产孕妇妊娠失败影响因素研究

Influencing factors of pregnancy failure in pregnant women with threatened abortion

目的探讨先兆流产患者妊娠失败的影响因素,为临床制定相关措施提供理论依据。方法选取2018年3月—2020年3月本院收治的200例先兆流产患者作为研究对象,根据其分娩情况分组,通过Logistic回归分析先兆流产患者妊娠失败的影响因素。结果研究对象中,妊娠良好组135例,妊娠不良组65例。单因素分析显示:不同年龄以及既往自然流产史、早期阴道流血、阴道大量流血、阴道炎、生殖道支/衣原体感染、子宫畸形、子宫肌瘤、黄体功能不全、宫内大血肿及孕期甲状腺功能低下情况不同的研究对象妊娠结局的差异具有统计学意义(均P<0.05)。Logistic回归分析结果显示:年龄≥35岁、既往有自然流产史、阴道大量流血、阴道炎、生殖道支/衣原体感染、子宫畸形、子宫肌瘤、黄体功能不全及宫内大血肿均是先兆流产患者妊娠失败的危险因素(均P<0.05)。结论影响先兆流产患者妊娠失败的危险因素较多,临床上对合并危险因素的先兆流产患者需采取一定的相关预防手段,以降低妊娠失败率。

Objective To investigate the influencing factors of pregnancy failure in patients with threatened abortion to pro-vide a theoretical basis for the formulation of clinical strategies.Methods A total of 200 patients with threatened abortion admitted to our hospital from March 2018 to March 2020 were selected as the study subjects and assigned to groups follow-ing their delivery status.Influencing factors of pregnancy failure in patients with threatened abortion were analyzed using Logistic regression.Results There were 135 patients in the good pregnancy group and 65 patients in the poor pregnancy group.Univariate analysis showed that there were significant differences in pregnancy outcomes among study subjects of dif-ferent ages as well as previous history of spontaneous abortion,early vaginal bleeding,massive vaginal bleeding,vaginitis,genital tract mycoplasma/chlamydia infection,uterine malformations,uterine fibroids,luteal phase dysfunction,large intra-uterine hematoma and hypothyroidism during pregnancy(all P<0.05).Logistic regression analysis showed that age≥35 years old,previous history of spontaneous abortion,massive vaginal bleeding,vaginitis,genital tract mycoplasma/chlamydia infection,uterine malformation,uterine fibroids,luteal phase dysfunction and large intrauterine hematoma were all risk factors for pregnancy failure in patients with threatened abortion(all P<0.05).Conclusion A host of risk factors affecting pregnancy failure remain in patients with threatened abortion.It is necessary to take certain preventive measures for patients with threatened abortion combined with risk factors to reduce the pregnancy failure rate in clinical practice.
