白细胞介素-8联合白细胞介素-10检测在血液疾病细菌感染中的临床意义

摘要 目的探究白细胞介素-8联合白细胞介素-10检测在血液疾病细菌感染中的应用价值。方法选取2019年9月至2020年5月本院收治的30例疑似血液疾病细菌感染患者作为研究对象,均给予抗感染等治疗及病原菌检测、细菌涂片和白细胞介素-8、白细胞介素-10水平检测,比较治疗前、治疗1个月后患者白细胞介素-8、白细胞介素-10水平,并根据临床疗效分为治愈组(n=20)、好转组(n=5)、恶化组(n=2);比较3组白细胞介素-8、白细胞介素-10水平;以病原菌检测、细菌涂片为金标准,分析白细胞介素-8联合白细胞介素-10在血液疾病细菌感染中的诊断效能(诊断符合率、灵敏度、特异度、误诊率、漏诊率、阳性预测值、阴性预测值)。结果病原菌检测、细菌涂片结果显示,30例患者中,确诊为细菌感染27例,非细菌感染3例。治疗后,27例患者中治愈20例,好转5例,恶化2例,治疗总有效率为92.59%。治疗后,患者白细胞介素-8、白细胞介素-10水平均低于治疗前,差异有统计学意义(P<0.05)。治愈组白细胞介素-8、白细胞介素-10水平均低于好转组、恶化组,且好转组低于恶化组,差异有统计学意义(P<0.05)。白细胞介素-8联合白细胞介素-10水平检测血液疾病细菌感染的诊断符合率为90.00%、灵敏度为92.59%、特异度为66.67%、误诊率为33.33%、漏诊率为7.41%、阳性预测值为96.15%、阴性预测值为50.00%。联合检测细菌感染的诊断灵敏度、符合率均高于白细胞介素-8、白细胞介素-10单独检测,差异有统计学意义(P<0.05),但联合检测与白细胞介素-8、白细胞介素-10单独检测诊断特异度、阳性预测值、阴性预测值比较差异无统计学意义。结论白细胞介素-8、白细胞介素-10水平能有效判断血液疾病细菌感染情况,二者联合检测诊断效能较为理想,可为疾病治疗、诊断、预后等提供指导。 Objective To explore the application value of interleukin-8 combined with interleukin-10 detection in bacterial infection of blood disease.Methods 30 patients with blood disease infection admitted to our hospital from September 2019 to May 2020 were selected as the study subjects,all of them were given anti infection treatment and pathogen detection,bacterial smear and interleukin-8 and interleukin-10 levels detection,the levels of interleukin-8 and interleukin-10 were compared before treatment and 1 month after treatment.There were divided into the cured group(n=20),the improved group(n=5)and the deteriorated group(n=2)according to the clinical efficacy,the levels of interleukin-8 and interleukin-10 were compared among the three groups;the diagnostic efficacy of interleukin-8 combined with interleukin-10 in bacterial infection of blood diseases(diagnostic coincidence rate,sensitivity,specificity,misdiagnosis rate,missed diagnosis rate,positive predictive value and negative predictive value)were analyzed with pathogen detection and bacterial smear as the gold standard.Results The results of pathogen detection and bacterial smear showed that among the 30 patients,27 cases were diagnosed as bacterial infections and 3 cases were non bacterial infections.After treatment,among the 27 patients,20 cases were cured,5 cases were improved and 2 cases were deteriorated,the total effective rate was 92.59%.After treatment,the levels of interleukin-8 and interleukin-10 in the patients were lower than those before treatment,and the difference was statistically significant(P<0.05).The levels of interleukin-8 and interleukin-10 in the cured group were lower than those in the improved group and the deteriorated group,and the improved group was lower than the deteriorated group,and the difference was statistically significant(P<0.05).The diagnostic coincidence rate of interleukin-8 combined with interleukin-10 for bacterial infection was 90.00%,the sensitivity was 92.59%,the specificity was 66.67%,the misdiagnosis rate was 33.33%,the missed diagnosis rate was 7.41%,the positive predictive value was 96.15%,and the negative predictive value was 50.00%.The diagnostic sensitivity and coincidence rate of the combined detection of bacterial infection were higher than those of interleukin-8 and interleukin-10 detection alone,and the difference was statistically significant(P<0.05),but there was no significant difference in diagnostic specificity,positive predictive value and negative predictive value between the combined detection and interleukin-8 and interleukin-10 detection alone.Conclusion The levels of interleukin-8 and interleukin-10 can effectively judge the bacterial infection of blood diseases,the combined detection and diagnosis of interleukin-8 and interleukin-10 have ideal diagnostic efficacy,and can provide guidance for disease treatment,diagnosis and prognosis.
