摘要
目的探讨床旁超声指导肺部机械辅助排痰治疗重症肺炎患儿的临床疗效。方法选取2019年1月至2021年1月入住本院儿童重症监护室(PICU)和/或呼吸科的100例重症肺炎患儿作为研究对象,按照随机数字表法分为两组,各50例。观察组采用床旁超声指导肺部机械辅助排痰治疗,对照组行人工叩背排痰治疗,比较两组治疗24、48、72 h后的心率、呼吸频率、动脉血二氧化碳分压(PCO_(2))、氧合指数及吸氧时间、入住PICU时间、总住院时间。结果治疗24、48、72 h后,观察组心率、呼吸频率均低于对照组,差异有统计学意义(P<0.05);治疗24、48、72 h后,观察组PCO_(2)均低于对照组,氧合指数均高于对照组,差异有统计学意义(P<0.05);观察组吸氧时间、入住PICU时间、总住院时间均短于对照组,差异有统计学意义(P<0.05)。结论床旁超声指导下肺部机械辅助排痰治疗重症肺炎患儿能有效维持患儿生命体征稳定,改善患儿血气指标,缩短吸氧时间及住院时间,促进患儿康复,值得临床推广应用。
Objective To explore the clinical effect of bedside ultrasound-guided pulmonary mechanical assisted sputum elimination therapy in the treatment of children with severe pneumonia.Methods 100 children with severe pneumonia who were admitted to the Pediatric Intensive Care Unit(PICU)and/or Respiratory department of our hospital from January 2019 to January 2021 were selected as the research subjects,and they were divided into two groups according to the random number table method,with 50 cases in each group.The observation group was treated with bedside ultrasound-guide pulmonary mechanical assisted sputum elimination treatment,while the control group was treated with manual back tapping to promote sputum elimination treatment.The heart rate,respiratory rate,arterial blood carbon dioxide partial pressure(PCO2),oxygenation index after 24,48 and 72 h of treatment,and oxygen inhalation time,PICU stay and total hospitalization time were compared between the two groups.Results After 24,48 and 72 h of treatment,the heart rate and respiratory rate of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05);after 24,48 and 72 h of treatment,the PCO2 of the observation group was lower than that of the control group,and the oxygenation index was higher than that of the control group,and the difference was statistically significant(P<0.05).The time of oxygen inhalation,PICU stay and total hospitalization time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Bedside ultrasound-guided pulmonary mechanical assisted sputum elimination can effectively maintain the stability of children's vital signs,improve their blood gas indexes,shorten the oxygen inhalation time and hospitalization time,and promote their rehabilitation,which is worthy of clinical application.
