地氟烷麻醉对成人患者心脏术后肺部并发症的影响

Effect of Desflurane Anesthesia on Pulmonary Complications after Cardiac Surgery in Adult Patients

目的:观察对比地氟烷与异丙酚麻醉对心脏手术患者术后肺部并发症(PPCs)的差异影响。方法:以2021年6月1日—12月31日到我院接受心脏外科手术的60名患者为本次研究目标,根据计算机生成的随机序列,将患者以1∶1的比例随机分为两组,分别为接受异丙酚麻醉的异丙酚组和接受地氟烷麻醉的地氟烷组,观察两组患者术后7d内和30d内出现PPCs的情况以及其他临床结局情况。结果:所有患者在心脏手术后7d发生PPCs的概率为20.0%,地氟烷组和异丙酚组患者在心脏手术后7d内以及30d内主要临床指标(PPCs的数量、严重程度等),以及术后其他并发症(如恶心呕吐)、再次气管插管概率、术后机械通气时间以及非计划ICU入院率和死亡率等次要结局指标均无显著差异(P>0.05)。结论:在心脏手术过程中使用地氟烷进行维持麻醉与使用异丙酚维持麻醉在术后出现PPCs及其严重程度上并无明显差异。

Objective:To observe and compare the difference of PPCs between desflurane and propofol anesthesia in patients undergoing cardiac surgery.Methods:60 patients who underwent cardiac surgery in our hospital from June 1 st 2021 to December 31 st 2021 were the objectives of this study.The patients were randomly divided into two groups in the ratio of 1∶1 according to the random sequence generated by the computer. The two study groups were propofol group in which patients received propofol anesthesia and desflurane group under desflurane anesthesia.The occurrence of PPCs and other clinical outcomes within 7 and 30 days after operation were observed.Results:The probability of PPCs in all patients 7 days after cardiac surgery was 20.0%. Within 7 and 30 days after operation, there was no significant difference between desflurane group and propofol group in the main clinical(indicators including the number and severity of PPCs),as well as other postoperative complications(such as nausea and vomiting), the probability of re-endotracheal intubation, the time of postoperative mechanical ventilation, unplanned ICU admission rate and mortality.Conclusion:There was no significant difference in PPCs and its severity between desflurane and propofol during cardiac surgery.
