多发性骨髓瘤患者外周血中TrxR-1表达水平与预后的相关性 被引量:5

Correlation between the Expression Level of TrxR-1in the Peripheral Blood of Patients with Multiple Myeloma and the Prognosis

摘要 目的:探讨多发性骨髓瘤(MM)患者外周血中硫氧还蛋白还原酶-1(TrxR-1)表达水平与预后的相关性。方法:选择2016年2月—2021年1月在我院血液科诊治的多发性骨髓瘤患者58例作为MM组,同期选择在本院非多发性骨髓瘤患者58例作为对照组。检测两组外周血TrxR-1表达水平,调查患者病理资料,随访患者的预后并进行相关性分析。结果:MM组的外周血TrxR-1相对表达水平高于对照组(P<0.05)。不同ISS分期与M蛋白分型的多发性骨髓瘤患者外周血TrxR-1表达水平对比差异都有统计学意义(P<0.05)。所有患者随访到2021年7月1日,平均随访时间为(29.38±1.58)个月,死亡18例,死亡率为31.0%,死亡患者的外周血TrxR-1表达水平高于生存患者(P<0.05)。Spearman分析显示外周血TrxR-1表达水平与预后死亡存在正相关性(r=0.682,P=0.000),Cox回归分析显示外周血TrxR-1表达水平为影响多发性骨髓瘤患者预后死亡的独立危险因素(P<0.05)。结论:TrxR-1在多发性骨髓瘤外周血表达明显升高,其高表达促进了多发性骨髓瘤的发生、发展,可作为预测多发性骨髓瘤预后的重要指标之一。 Objective:To investigate the correlation between the expression of thioredoxin reductase-1(TrxR-1) in peripheral blood of patients with multiple myeloma(MM) and the prognosis.Methods:From February 2016 to January 2021, 58 cases of patients with multiple myeloma who were diagnosed and treated in our hospital were selected as the MM group, and 58 cases of patients with non-multiple myeloma in our hospital at the same time were selected as the control group. The expression level of TrxR-1 in the peripheral blood of the two groups were detected, the pathological data of the patients were investigated, the prognosis of the patients were followed up and the correlation analysis were carried out.Results:The relative expression level of TrxR-1 in peripheral blood of the MM group were higher than that of the control group(P<0.05).There were statistically significant differences in the expression level of TrxR-1 in the peripheral blood of patients compared with multiple myeloma of different ISS stages and M protein types(P<0.05).All patients were followed up until July 1 st 2021. The average follow-up time were(29.38±1.58) months, there were 18 cases died, and the mortality rate were 31.0%. The TrxR-1 levels in the peripheral blood of the dead patients were higher than that of the surviving patients(P<0.05).Spearman analysis showed that the TrxR-1 levels in peripheral blood were positively correlated with prognostic death(r=0.682,P=0.000). Cox regression analysis showed that the TrxR-1 levels in peripheral blood were independent risk factors affected the prognostic death of patients with multiple myeloma(P<0.05).Conclusion:The expression of TrxR-1 in the peripheral blood of multiple myeloma are significantly increased. Its high expression promotes the occurrence and development of multiple myeloma and can be used one of the important indicators for predicting the prognosis of multiple myeloma.
