腹腔镜手术联合GnRH-a治疗早期子宫内膜异位症临床疗效分析

Clinical efficacy of laparoscopic surgery combined with GnRH-a in the treatment of early endometriosis

目的子宫内膜异位症严重影响着育龄期女性的生活质量,新型腹腔镜手术联合GnRH-a的治疗方法能更好缓解患者症状,增加妊娠率并减少复发。方法选择采用新型腹腔镜手术方式治疗的102例患者进行回顾性分析,根据患者术后是否联合应用促性腺激素释放激素激动剂(GnRH-a)分为实验组和对照组,治疗结束后随访1年,比较2组患者治疗后的临床疗效,包括治疗后总的有效率、糖类抗原125(CA125)、促卵泡激素(FSH)、雌二醇(E_(2))以及治疗后妊娠率及复发率。结果在子宫内膜异位症疾病早期采用新型腹腔镜手术方式联合GnRH-a组患者术后总有效率(94.2%)明显高于对照组(86%),差异有统计学意义(P<0.05);客观指标CA125、FSH及雌二醇均较对照组明显降低,差异有统计学意义(P<0.05);治疗后1年内妊娠率实验组(53.8%)较对照组(34%)明显高,实验组1年内无复发患者,对照组患者术后有2例复发,复发率4%,实验组较对照组明显低,差异有统计学意义(P<0.05)。结论在子宫内膜异位症早期采用新型腹腔镜手术方式联合GnRH-a这种治疗方法可以缓解患者临床症状,增加患者妊娠率,并可明显减低复发率,是一种有效的治疗子宫内膜异位症的方法,值得临床医生采纳。

Objective To find a new treatment of laparoscopic surgery combined with GnRH-a,which can be better to relieve the symptoms of patients,increase the pregnancy rate and reduce the recurrence.Methods A total of 102 patients were treated by the new laparoscopic surgery and retrospectively analyzed.According to whether the patients were combined with GnRH-a after surgery,the patients were divided into the experimental group and the control group.After treatment,the patients were followed up for one year,and the clinical efficacy of the two groups was compared.Including the total effective rate,CA125,FSH and E_(2),pregnancy rate and recurrence rate.Results The total effective rate of experimental group(94.2%)was significantly higher than that of the control group(86%)(P<0.05).Objective indexes such as CA125,FSH and E_(2) were significantly lower than those in the control group(P<0.05).Within one year after treatment,the pregnancy rate(53.8%)was significantly higher than that of the control group(34%).There was no recurrence in the experimental group and there were two cases of recurrence in the control group.Conclusion In the early stage of endometriosis,the new laparoscopic surgery combined with GnRH-a can relieve the clinical symptoms of patients,increase the pregnancy rate and reduce the recurrence rate.It is an effective treatment for endometriosis.
