张家口市男男性行为者人乳头瘤病毒感染情况调研 被引量:2

Investigation on human papilloma virus infection among men who have sex with men in Zhangjiakou city

摘要 目的 调查张家口地区男男性行为者(MSM)人乳头瘤病毒(HPV)感染现况及危险因素,为进一步预防该人群HPV感染提供科学依据。方法 招募MSM进行调查,采集肛门拭子标本行HPV分型检测,比较不同特征HPV阳性检出率的差异。采用Logistic回归模型分析HPV感染危险因素。结果 张家口地区MSM感染率最高的亚型为6型、11型、16型、58型、52型和33型,以单一亚型感染为主;各年龄段患者HPV总阳性检出率比较,差异具有统计学意义(P<0.05),18~24岁检出率最高。单一感染类型的阳性检出率18~24岁与45~54岁比较,差异具有统计学意义(P<0.05)。双重感染类型的阳性检出率在45~54岁比例最高,差异具有统计学意义(P<0.05)。三重感染类型的阳性检出率在≥55岁比例最高,差异具有统计学意义(P<0.05)。不同学历患者总阳性检出率比较,差异具有统计学意义(P<0.05),初中阳性检出率最高为60.71%。多因素Logistic回归分析显示,18~24岁、45~54岁、小学学历、初中学历、高中学历、初次性行为年龄≤19岁、自诉有性病史都是HPV感染的危险因素。结论 张家口地区MSM的HPV感染率较高,感染亚型分布与其他地区存在差异,以单一感染为主,应加强预防HPV感染的健康教育,有必要将年轻MSM纳入HPV 9价疫苗接种计划,并在他们发生性行为之前进行疫苗接种。 Objective To investigate the status and risk factors of human papilloma virus(HPV) infection among men who have sex with men(MSM) in Zhangjiakou area,so as to provide scientific basis for further prevention of HPV infection in this population.Methods MSM were recruited for investigation.The HPV typing test was conducted by collecting anal swab samples.Rank sum test was used to compare the difference of HPV positive rates among different characteristic groups.Logistic regression model was used to analyze the risk factors of HPV infection.Results The subtypes with the higher infection rates of MSM in Zhangjiakou area were type 6,type 11,type 16,type 58,type 52 and type 33,and single subtype infection was dominant.There were statistical differences in the total positive detection rate of patients among different ages(P<0.05),and the detection rate was the highest in the 18-24 years old.The positive detection rates of single infection type were higher in 18-24 years old and 45-54 years old,and the difference was statistically significant(P<0.05).The positive detection rate of the double infection type was the highest in 45-54 years old,and the difference was statistically significant(P<0.05).The positive detection rate of triple infection type was the highest in ≥55 years old,and the difference was statistically significant(P<0.05).There was a statistical difference in the total positive detection rate among patients with different academic qualifications(P<0.05),and the highest positive detection rate in junior high school is 60.71%.Multivariate Logistic regression analysis showed that 18-24 years old,45-54 years old,primary school,junior high school,senior high school,age of initial sexual behavior ≤19 years old,private venereal diseases history were the risk factors for HPV infection.Conclusions The HPV infection rate was higher among MSM in Zhangjiakou area.The distribution of subtypes in this region is different from other regions,with single infection as the main.Health education on prevention of HPV infection should be strengthened.It is necessary to include young MSM in HPV9-valent vaccination plan and provide vaccination before their sexual behavior.
