新冠疫情闭环管理中医务人员心理应激反应及相关因素研究

Psychological stress reactions and related factors in medical staff under closed-loop management during COVID-19 epidemic

目的 了解新冠疫情闭环管理期间医务人员的心理应激反应及焦虑情绪现状并分析相关因素,探讨社会支持的保护作用,为调控其情绪应激、解除压力源提供参考依据。方法 使用在线调查平台创建问卷,于2020年11月26日—30日,对上海某三级闭环管理医院医务人员进行调查,采用自编一般资料问卷、修订版事件影响量表(IES-R)、九项健康问卷(GHQ-9)和社会支持评定量表问卷(SSRS)进行调查。收回有效问卷960份,有效率94.44%。结果 医护人员在闭环管理期间IES-R平均分为(17.08±12.84)分;PHQ-9平均分为(5.14±5.09)分;SSRS总平均分为(43.88±8.29)分,主观支持、客观支持、支持利用度3个因子得分分别为(11.19±3.48)、(24.93±5.12)、(7.74±1.83)分。多重线性回归分析显示,IES-R总分及PHQ-9总分与主观支持以及支持利用度负向相关(P<0.001),与客观支持同样负向相关(P<0.05)。结论 在医院闭环管理期间,医护人员无论是心理应激反应还是焦虑得分都较高。护士相比医生与行政人员而言,更可能出现情绪问题。社会支持水平高是医护人员出现应激反应以及焦虑情绪的保护因素。

Objective To explore the status and related factors among medical staff under closed-loop management during the COVID-19 epidemic,analyzing the related factors,and explore the protective effect of social support.So as to provide a reference for regulating the emotional stress of medical staff.Methods A survey was conducted among 960 medical staff who were under closed-loop management.They assessed the psychological stress response,mental health and social support with the revised Event Impact Scale(IES-R),Patient Health Questionnaire(PHQ-9) and Social Support Rating Scale(SSRS).Results The average IES-R and PHQ-9 score of 960 medical staff was( 17.08±12.84) and(5.14±5.09).The average SSRS score was(43.88±8.29) The scores of subjective support,objective support,and support utilization were(11.19±3.48),(24.93±5.12),and(7.74±1.83),respectively.Multiple linear regression analysis showed that the total scores of IES-R and PHQ-9 were negatively correlated with subjective support and support utilization(P<0.001),and also negatively correlated with objective support(P<0.05).Conclusion During the period of closed-loop management in hospitals,the scores of psychological stress and anxiety of medical staff are higher.Nurses are more prone to severe psychological stress than other medical staff.The high level of social support is the protective factor for medical staff’s stress reaction and anxiety.
