滋肾祛瘀养膜方联合雷火灸预防宫腔粘连分离术后再粘连临床研究

Clinical Study of Decoction for Nourishing Kidney and Removing Blood Stasis and Nourishing Endometrium Combined with Thunder Fire Moxibustion on Preventing Re-adhesion After Separation of Intrauterine Adhesion

目的:观察滋肾祛瘀养膜方联合雷火灸预防宫腔粘连分离术后再粘连的临床疗效。方法:选择80例行宫腔粘连分离术的患者为研究对象,按照随机数字表法分为对照组和观察组,每组各40例。对照组给予滋肾祛瘀养膜方及雌二醇地屈孕酮片治疗,观察组在对照组治疗的基础上联合雷火灸治疗。比较两组患者的临床疗效及治疗前后宫腔粘连评分、子宫内膜厚度、子宫动脉血流指标及中医证候积分变化情况。结果:两组患者治疗后宫腔粘连评分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后子宫内膜厚度大于本组治疗前,且治疗后观察组大于对照组,差异具有统计学意义(P<0.05);两组患者治疗后搏动指数、血管阻力指数低于本组治疗前,且观察组低于对照组,但差异无统计学意义(P>0.05)。观察组有效率为67.5%,对照组有效率为57.5%,两组患者有效率比较,差异有统计学意义(P<0.05)。结论:滋肾祛瘀养膜方联合雷火灸能够改善宫腔粘连患者术后的临床症状,增加子宫内膜厚度,促进子宫内膜的血液循环,并且可预防宫腔再次粘连。

Objective:To observe the clinical efficacy of Nourishing Kidney and Removing Blood Stasis and Nourishing Endometrium combined with thunder fire moxibustion on preventing re-adhesion after separation of intrauterine adhesion(IUA).Methods:A total of 80 patients who received separation of IUA were divided into the control group and the observation group according to random number table,with 40 cases in each group.The control group was treated with Decoction for Nourishing Kidney and Removing Blood Stasis and Nourishing Endometrium combined with Estradiol Didroxyprogesterone Tablets,while the observation group was additionally treated with thunder fire moxibustion on the basis of the control group.The clinical efficacies,the changes of uterine cavity adhesion score,endometrial thickness,uterine artery blood flow and TCM syndrome scores of the two groups before and after treatment were compared.Results:The uterine cavity adhesion score of the two groups after treatment was lower than that of the same group before treatment,and the observation group′s was lower than that of the control group after treatment,and the difference was statistically significant(P0.05).The effective rate of the observation group was 67.5%,and that of the control group was 57.5%.The difference between the two groups was statistically significant(P<0.05).Conclusion:Decoction for Nourishing Kidney and Removing Blood Stasis and Nourishing Endometrium combined with thunder fire moxibustion can improve the clinical symptoms of patients with IUA after operation,increase the thickness of endometrium,promote the blood circulation of endometrium,and prevent IUA from occurring again.
