中医闭合手法复位治疗踝部骨折临床观察

Clinical Observation on Traditional Chinese Medicine Closed Manipulative Reduction in the Treatment of Ankle Fracture

目的分析中医闭合手法复位治疗踝部骨折的临床效果。方法选择南昌市洪都中医院2018年9月—2020年9月收治的70例踝部骨折患者作为研究对象,根据其入院治疗时间分组,将2018年9月—2019年8月入院治疗的患者纳入参照组(35例),将2019年9月—2020年9月入院治疗的患者纳入研究组(35例),2组分别实施手术切开复位治疗、中医闭合手法复位治疗。观察和比较2组患者复位优良率、踝部疼痛缓解时间、踝部肿胀缓解时间和治疗前后踝关节功能评分。结果研究组复位优良率94.28%(33/35)高于参照组77.14%(27/35),2组比较差异有统计学意义(P<0.05)。研究组治疗前踝关节功能评分与参照组比较,差异无统计学意义(P>0.05);2组治疗后的踝关节功能评分低于治疗前,研究组治疗后的踝关节功能评分低于参照组,差异有统计学意义(P<0.05);且研究组踝部疼痛缓解时间、踝部肿胀时间短于参照组,差异有统计学意义(P<0.05)。结论采用中医闭合手法复位治疗踝部骨折可有效提高复位优良率,还能促进患者复位后踝关节功能的改善,缩短患者踝部疼痛缓解时间和肿胀缓解时间。

Objective To analyze the clinical effect of closed manipulative reduction in the treatment of ankle fracture.Methods 70 cases of ankle fracture patients in Nanchang Hongdu Hospital of Traditional Chinese Medicine from September 2018 to September 2020 were selected as the research objects,and they were divided into two groups according to their different hospitalization time.The patients admitted from September 2018 to August 2019 were included in the control group(35 cases),and the patients admitted from September 2019 to September 2020 were included in the study group(35 cases).The two groups were treated with open reduction and traditional Chinese medicine closed manual reduction treatment.The excellent and good reduction rate,ankle pain relief time,ankle swelling relief time and ankle function score before and after treatment were observed and compared between the two groups.Results The excellent and good reduction rate of the study group was 94.28%(33/35)higher than that of the reference group 77.14%(27/35),and the difference was statistically significant(P<0.05).There was no significant difference in ankle function score between the study group and the reference group before treatment(P>0.05).The ankle function score of the two groups after treatment was lower than that before treatment,and the ankle function score of the study group after treatment was lower than that of the reference group,and the difference was statistically significant(P<0.05).The pain relief time and ankle swelling time of the study group were shorter than those of the reference group,and the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine closed manipulative reduction in the treatment of ankle fractures can effectively improve the excellent and good rate of reduction,promote the improvement of ankle function after reduction,and shorten the time of ankle pain relief and swelling relief.
