中药汤剂结合情志护理干预对先兆流产患者妊娠结局的临床研究

Clinical Study on Chinese Medicine Decoction Combined with Emotional Nursing Intervention on the Pregnancy Outcome of Threatened Abortion

目的研究先兆流产诊疗中中药汤剂结合情志护理干预对患者妊娠结局的影响。方法回顾性选取2020年2月—2021年2月医院收治的100例先兆流产患者,依据诊疗方法分为中药汤剂结合情志护理干预组(结合治疗组)和常规西医治疗组(常规治疗组),各50例,比较2组患者的临床疗效、中医证候积分、症状改善时间、妊娠结局及满意度等。结果结合治疗组患者治疗的总有效率高于常规治疗组(P<0.05)。诊疗后,结合治疗组患者的SCL-90评分、主症积分、次症积分、总积分均低于常规治疗组(P<0.05)。结合治疗组患者的腹痛缓解时间、腰痛缓解时间、止血时间均短于常规治疗组(P<0.05)。结合组患者的顺产率高于常规治疗组(P<0.05),剖宮产率低于常规治疗组,2组流产率、活产率、妊娠并发症及产后并发症发生情况比较,差异均无统计学意义(P>0.05)。结合治疗组患者的入院指导、出院指导、生活护理、护理技能、服务态度评分均高于常规治疗组(P<0.05)。结论先兆流产诊疗中中药汤剂结合情志护理干预较常规西医治疗更能有效改善患者妊娠结局。

Objective To study the effect of Chinese medicine decoction combined with emotional nursing intervention on the pregnancy outcome of threatened abortion.Methods A retrospective selection of 100 patients with threatened abortion in our hospital from February 2020 to February 2021 were divided into Chinese medicine decoction combined with emotional nursing intervention group(combined treatment group)and conventional western medicine treatment group(conventional treatment group)according to the diagnosis and treatment methods,with 50 cases in each group.The clinical effect,traditional Chinese medicine(TCM)syndrome score,symptom improvement time,pregnancy outcome and satisfaction were compared between the two groups.Results The total effective rate of combined treatment group was higher than that of conventional treatment group(P<0.05).After diagnosis and treatment,the SCL-90 score,main symptom score,secondary symptom score,and total score of the combined treatment group were lower than those of the conventional treatment group(P<0.05).The abdominal pain relief time,back pain relief time and hemostasis time of combined treatment group were shorter than those of conventional treatment group(P<0.05).The vaginal delivery rate of the combined group was higher than that of the conventional treatment group(P<0.05),and the cesarean section rate was lower than that of the conventional treatment group.There was no significant difference in the abortion rate,live birth rate,pregnancy complications and postpartum complications between the two groups(P>0.05).The scores of admission guidance,discharge guidance,life care,nursing skills and service attitude of patients in combined treatment group were higher than those in conventional treatment group(P<0.05).Conclusion Chinese medicine decoction combined with emotional nursing intervention in the diagnosis and treatment of threatened abortion is more effective in improving the pregnancy outcome of patients than conventional western medicine treatment.
