CAPD患者肌少症的发生情况及相关因素分析

Analysis of the occurrence and related factors of sarcopenia in patients with CAPD

目的探讨持续非卧床腹膜透析(CAPD)患者肌少症与非肌少症发生情况及危险因素。方法选取2019年8月至2020年8月规律接受CAPD治疗时间>3个月、资料完整患者123例。采用主观综合营养评估(subjective global assessment,SGA)法评估患者营养状况,肌少症的诊断采用最新EWGSOP 2法评价,并收集患者的一般情况及血生化指标。按照有无肌少症分为肌少症组(25例)和无肌少症组(98例),对比2组患者临床资料和生化指标情况,应用二元Logistic回归模型法探讨影响CAPD患者肌少症的相关因素。结果123例CAPD患者中存在肌少症25例,患病率为20.33%。2组患者的年龄、透析龄、SGA、HGS、LTI、FTI、E/I、BMI、ALB、25(OH)D、hs-CPR、GLU差异有统计学意义(P<0.05),而性别、HBG、TCH、TG、Ca、P、PTH、RRF2组间差异无统计学意义(P>0.05)。二元Logistic回归分析结果显示,年龄、透析龄、SGA、HGS、LTI、BMI、hs-CPR是腹透患者发生肌少症的相关因素。结论腹透患者肌少症比较多见,年龄、透析龄、SGA、HGS、LTI、BMI、hs-CRP是腹透患者发生肌少症的相关因素。

Objective To investigate the occurrence and risk factors of sarcopenia and non-sarcopenia in patients with Continuous Ambulatory Peritoneal Dialysis(CAPD).Methods A total of 123 patients with were treated by CAPD for>3 months in our hospital from August 2019 to August 2020 were enrolled in the study.The nutritional status of patients was assessed by Subjective Global Assessment(SGA),and the diagnosis of sarcopenia was evaluated by the latest EWGSOP 2 method,and the general situation and blood biochemical indicators of patients were collected.Moreover the patients with sarcopenia were divided into sarcopenia group(n=25)and non-sarcopenia group(n=98)according to the presence or absence of sarcopenia.The clinical data and biochemical parameters were observed and compared between the two groups.In addition the related factors of sarcopenia in patients with CAPD were analyzed by binary Logistic regression model.Results Of the 123 patients with CAPD patients,there were 25 cases of sarcopenia,with the prevalence rate being 20.33%.There were significant differences in patient’s age,dialysis duration,and the levels of SGA,HGS,LTI,FTI,E/I,BMI,ALB,25(OH)D,hs-CPR and GLU between the two groups(P<0.05).However there were no significant differences in patient’s gender,and the levels of HBG,TCH,TG,Ca,P,PTH and RRF between the two groups(P>0.05).Binary Logistic regression analysis showed that age,dialysis duration,SGA,HGS,LTI,BMI,and HS-CPR were the related factors with the occurrence of sarcopenia in patients with peritoneal dialysis.Conclusion Sarcopenia is more common in patients with peritoneal dialysis,and age,dialysis duration,SGA,HGS,LTI,BMI and Hs-CRP are the related factors with the occurrence of sarcopenia in patients with peritoneal dialysis.
