快速康复外科理念联合心理护理对髋关节置换患者预后的影响

Influence of Fast Recovery Surgery Concept Combined with Psychological Nursing on Prognosis of Patients with Hip Replacement

目的:分析髋关节置换患者接受快速康复外科联合心理护理的临床效果。方法:选取2020年1-12月钟祥市人民医院骨科收治的髋关节置换患者65例,随机分为两组,对照组33例,实施常规护理;观察组32例,实施快速康复外科联合心理护理。观察两组患者抑郁自评量表(SDS)评分、焦虑自评量表(SAS)评分、视觉模拟评分(VAS)、围手术期指标及并发症发生率。结果:护理前,两组SDS、SAS评分比较,差异无统计学意义(P>0.05);护理后,观察组SDS、SAS评分均低于对照组,差异有统计学意义(P<0.05);术前,两组VAS评分比较,差异无统计学意义(P>0.05);术后第1天、术后第1次下床、出院时,观察组VAS评分低于对照组,差异有统计学意义(P<0.05);观察组术中出血量少于对照组,早期下床活动时间、术后拆线时间、住院时间均短于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:对髋关节置换患者实施快速康复外科联合心理护理,可以改善患者心理状态,降低患者术后疼痛程度,减少术中出血量及并发症的发生,缩短病程,值得推广。

Objective: To analyze the clinical influence of fast recovery surgery combined with psychological nursing for hipreplacement patients. Methods: A total of 65 hip replacement patients admitted to the Department of Orthopedics of ZhongxiangPeople’s Hospital from January 2020 to December 2020 were selected, and were randomly divided into two groups, the controlgroup of 33 cases received routine nursing, the observation group of 32 cases received fast recovery surgery combined withpsychological nursing. The Self-rating Depression Scale(SDS), Self-rating Anxiety Scale(SAS), Visual Analogue Scale(VAS),perioperative indicators and complication rates in the two groups were observed. Results: Before nursing, there was no significantdifference in SDS and SAS scores between the two groups(P>0.05). After nursing, the SDS and SAS scores of the observationgroup were lower than those of the control group, and the difference was statistically significant(P<0.05). Before surgery, there wasno significant difference in VAS scores between the two groups(P>0.05). The VAS score of the observation group was lower thanthat of the control group on the 1 st day after operation, the first time to get out of bed and discharge after operation, and thedifference was statistically significant(P<0.05). The intraoperative blood loss in the observation group was less than that in thecontrol group, and the early ambulation time, postoperative suture removal time, and hospital stay were shorter than those in thecontrol group, and the differences were statistically significant(P<0.05). The incidence of complications in the observation groupwas lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion: Fast recoverysurgery combined with psychological nursing for hip replacement patients can improve the patient’s psychological state, reducepostoperative pain, reduce intraoperative blood loss and complications, and shorten the course of the disease, which is worthy ofpromotion.
