加速康复外科理念在多发肋骨骨折术后胸腔引流管管理中的应用效果观察

Observation on the Application Effect of Enhanced Recovery Surgery Concept in the Management of Thoracic Drainage Tube after Multiple Rib Fractures

目的:分析加速康复外科理念对多发肋骨骨折术后胸腔引流管管理的影响。方法:选取2018年10月-2020年10月长沙市中医医院收治的多发肋骨骨折患者88例,随机分为两组,各44例。观察组实施加速康复外科护理管理,对照组实施常规管理。比较两组患者术前、术后3、4、5 d的疼痛情况、胃肠功能恢复时间及术后胸腔引流管留置时间。结果:术前、术后3 d,两组视觉模拟评分法评分比较,差异无统计学意义(P>0.05)。观察组术后4 d、5 d视觉模拟评分均低于对照组,差异有统计学意义(P<0.05)。观察组术后胸腔引流管留置时间及胃肠功能恢复时间均短于对照组,差异有统计学意义(P<0.05)。结论:加速康复外科理念下术后胸腔引流管护理管理,能减轻多发肋骨骨折患者术后疼痛,缩短术后胸腔引流管留置时间及胃肠功能恢复时间。

Objective: To analyze the effect of enhanced recovery surgery concept in the management of thoracic drainage tube after multiple rib fractures. Methods: A total of 88 patients with multiple rib fractures admitted to Changsha Traditional Chinese Medicine Hospital from October 2018 to October 2020 were selected and randomly divided into two groups, with 44 cases in each group. The observation group was given enhanced recovery surgery nursing management, while the control group was given routine management. The pain condition, gastrointestinal function recovery time and postoperative thoracic drainage tube indwelling time were compared between the two groups before operation, 3, 4, and 5 days after operation. Results: There was no significant difference between the two groups in the visual analog scale before and 3 days after the operation(P>0.05). The visual analog scores of the observation group at 4 days and 5 days after operation were lower than those of the control group, and the difference was statistically significant(P<0.05). The postoperative thoracic drainage tube indwelling time and gastrointestinal function recovery time in the observation group were shorter than those in the control group, and the difference was statistically significant(P<0.05). Conclusion: Nursing and management of postoperative thoracic drainage tube under the concept of enhanced recovery surgery can reduce postoperative pain in patients with multiple rib fractures, and shorten postoperative thoracic drainage tube indwelling time and gastrointestinal function recovery time.
